摘要
膝骨关节炎是一种以膝关节疼痛、畸形、活动受限为主要临床表现的慢性退行性关节疾病。滑膜炎是膝骨关节炎中主要的危险因素之一,滑膜的炎症反应会导致软骨细胞损伤和关节软骨退变等病理改变,进而导致膝骨关节炎的发生。巨噬细胞是膝关节滑膜组织中重要的免疫细胞,可以有效维持关节内部微环境的稳态,活化的巨噬细胞受到相关细胞因子刺激后可极化为M1促炎表型和M2抗炎表型,M1促炎表型的巨噬细胞会分泌相关促炎细胞因子,从而使关节内部产生炎症微环境,关节内部的炎症反应最终会导致关节软骨的降解和退化。中医药治疗目前仍是我国临床治疗膝骨关节炎的主要药物选择。文章以中药单体和中药复方调控膝骨关节炎中滑膜巨噬细胞的极化为角度,系统检索了相关机制研究并进行筛选总结,分析中医药调控膝骨关节炎中巨噬细胞极化的相关研究进展,为膝骨关节炎的药物治疗提供了新的思路。
Knee osteoarthritis is a chronic degenerative joint disorder mainly manifested by knee pain,deformity,and restricted movement.Synovitis is one of the most crucial risk factors in knee osteoarthritis.The inflammatory reaction of the synovium can result in pathological alterations such as chondrocyte injury and articular cartilage degeneration,thereby causing the occurrence of knee osteoarthritis.Macrophages are significant immune cells in the synovial tissue of the knee joint and play a vital role in maintaining the homeostasis of the internal microenvironment of the joint.Activated macrophages can be polarized into M1 pro-inflammatory and M2 anti-inflammatory phenotypes upon stimulation by related cytokines.The M1 pro-inflammatory phenotype macrophages secrete relevant pro-inflammatory cytokines,thereby generating an inflammatory microenvironment within the joint.The inflammatory response within the joint eventually leads to the degradation and deterioration of articular cartilage.Currently,traditional Chinese medicine treatment remains one of the main pharmaceutical options for clinical management of knee osteoarthritis in China.Therefore,this review,from the perspective of the regulation of synovial macrophage polarization in knee osteoarthritis by single-component and compound traditional Chinese medicines,systematically searches for and screens relevant mechanism studies for summary,analyzes the research progress of traditional Chinese medicine in regulating macrophage polarization in knee osteoarthritis,providing novel ideas for the pharmacological treatment of knee osteoarthritis.
