摘要
目的探讨强化人文关怀联合主动保温在腹腔镜膀胱全切尿流改道术中的应用效果。方法选取2023年11月—2024年6月在聊城市人民医院行腹腔镜膀胱全切尿流改道术的54例患者作为研究对象,按照随机数字表法分为观察组和对照组各27例,对照组实施常规人文关怀联合被动保温护理,观察组在对照组的基础上实施强化人文关怀联合主动保温护理,比较2组患者的人文关怀满意度和术中入手术间后即刻(T_(1))、手术切皮时(T_(2))、手术开始后2 h(T_(3))、手术开始后4 h(T_(4))、手术结束时(T_(5))、麻醉恢复期(T_(6))6个时点的体温监测数值与低体温发生率。结果观察组术前访视、术前转运、等候手术、术中、麻醉复苏、术后转运、术后回访7个维度的人文关怀满意度单项得分以及总分高于对照组,差异有统计学意义(P<0.001)。观察组术中T_(1)、T_(2)、T_(3)、T_(4)、T_(5)、T_(6)各时点体温高于对照组,差异有统计学意义(P<0.05)。观察组低体温发生率为0,低于对照组的29.63%,差异有统计学意义(P<0.05)。结论强化人文关怀联合主动保温可提高腹腔镜膀胱全切尿流改道术后患者对手术室护理的人文关怀满意度,降低低体温发生率。
Objective To explore the application effect of strengthening humanistic care combined with active thermal insulation in laparoscopic total cystectomy for urinary diversion.Methods A total of 54 patients who underwent laparoscopic total cystectomy for urinary diversion in Liaocheng People's Hospital from November 2023 to June 2024 were selected as the research subjects.They were divided into an observation group and a control group,with 27 cases in each group,according to the random number table method.Conventional humanistic care combined with passive insulation nursing was administered to the control group.On the basis of the care given to the control group,strengthening humanistic care combined with active thermal insulation nursing was implemented for the observation group.The satisfaction of humanistic care and the temperature monitoring values and the incidence of hypothermia at six time points during the operation,namely,immediately after entering the operating room during the operation(T_(1)),at the time of skin incision during the operation(T_(2)),2 hours after the start of the operation(T_(3)),4 hours after the start of the operation(T_(4)),at the end of the operation(T_(5)),and during the anesthesia recovery period(T_(6)),were compared between the two groups of patients.Results The individual scores and total scores of humanistic care satisfaction in the seven dimensions of preoperative visit,preoperative transfer,waiting for surgery,intraoperative,anesthesia recovery,postoperative transfer and postoperative followup in the observation group were higher than those of the control group,the differences were statistically significant(P<0.001).The temperature at time points T_(1),T_(2),T_(3),T_(4),T_(5) and T_(6) during the operation in the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The incidence of hypothermia in the observation group was 0,which was lower than 29.63%in the control group,and the difference was statistically significant(P<0.05).Conclusion Strengthening humanistic care combined with active thermal insulation can improve the satisfaction of patients with humanistic care in operating room nursing after laparoscopic total cystectomy for urinary diversion,and reduce the incidence of hypothermia.
