生化检验在糖尿病肾病早期诊断中的应用价值评估

摘要 目的探讨生化检验在糖尿病肾病早期诊断中的临床价值。方法本研究采用回顾性分析方法,选取2023年4月—2024年7月湖北省直属机关医院收治的90例糖尿病肾病患者作为试验组。同期另选取90例无肾病并发症的糖尿病患者作为对照组。所有受试者均接受血液和尿液生化指标检测,比较2组患者的空腹血糖(fasting blood glucose,FBG)、血肌酐(serum creatinine,sCr)、糖化血红蛋白(glycated hemoglobin A1c,HbA1c)和尿微量白蛋白(urinary microalbumin,mAlb)水平。采用受试者工作特征(receiver operating characteristic,ROC)曲线分析上述指标对糖尿病肾病的诊断价值。结果试验组患者的FBG为(13.62±1.25)mmol/L,高于对照组的(10.28±1.34)mmol/L;试验组患者的sCr为(175.58±10.15)μg/L,高于对照组的(112.20±10.23)μg/L;试验组患者的HbA1c为(9.52±1.02)%,高于对照组的(7.16±1.08)%;试验组患者的mAlb为(46.29±6.23)mg/L,高于对照组的(6.24±1.18)mg/L,差异均有统计学意义(P<0.001)。ROC曲线分析显示,FBG、sCr、HbA1c、mAlb诊断糖尿病肾病的曲线下面积(area under the curve,AUC)分别为0.894、0.870、0.879和0.875(P<0.001)。结论血液与尿液联合生化检验在糖尿病肾病早期诊断中展现出良好的诊断效能,有助于疾病的早期评估,并可为后续临床治疗方案的制定提供可靠依据。 Objective This investigation sought to delineate the diagnostic significance of biochemical assays in the incipient stages of diabetic nephropathy.Methods A retrospective cohort was assembled,comprising 90 individuals with clinically confirmed diabetic nephropathy admitted to the Affiliated Hospital of Hubei Provincial Government between April 2023 and July 2024,designated as the experimental group.An additional 90 patients diagnosed with diabetes mellitus but without nephropathy were enrolled as the control group.All participants underwent comprehensive hematological and urinary biochemical assessments.Indices including fasting plasma glucose(FBG),serum creatinine(sCr),glycated hemoglobin A1c(HbA1c),and urinary microalbumin(mAlb)were quantified and compared between groups.Receiver operating characteristic(ROC)curve analyses were employed to appraise their discriminative capacity for nephropathy.Results The FBG of patients in the experimental group was(13.62±1.25)mmol/L,which was higher than(10.28±1.34)mmol/L in the control group.The sCr o f p a t i e n t s i n t h e experimental group was(175.58±10.15)μg/L,which was higher than(112.20±10.23)μg/L in the control group.The HbA1c of patients in the experimental group was(9.52±1.02)%,which was higher than(7.16±1.08)%in the control group.The mAlb of patients in the experimental group was(46.29±6.23)mg/L,which was higher than(6.24±1.18)mg/L in the control group.All differences were statistically significant(P<0.001).ROC curve analysis showed that the area under the curve(AUC)for FBG,sCr,HbA1c,and mAlb in diagnosing diabetic nephropathy were 0.894,0.870,0.879,and 0.875,respectively(P<0.001).Conclusion The integration of bloodand urine-based biochemical markers offers substantial diagnostic performance in the early recognition of diabetic nephropathy.Such combined profiling facilitates timely risk stratification and provides a robust foundation for individualized therapeutic planning.
