输尿管软镜碎石术与经皮肾镜碎石术对肾和输尿管上段结石患者围手术期指标及炎症因子水平的影响

Effects of flexible ureteroscopic lithotripsy and percutaneous nephrolithotomy on perioperative indexes and inflammatory factor levels in patients with renal and upper ureteral calculus

摘要目的探究输尿管软镜碎石术与经皮肾镜碎石术对肾和输尿管上段结石患者围手术期指标及炎症因子水平的影响。方法回顾性分析2021年1月至2022年12月鹰潭市人民医院泌尿外科100例肾和输尿管上段结石患者的临床资料,以不同手术方案分为A组与B组,各50例。A组实施输尿管软镜碎石术治疗,B组实施经皮肾镜碎石术治疗,比较两组手术相关指标、结石清除率、感染情况、肾功能指标[血清肌酐(serum creatinine,SCr)、尿素氮(urea nitrogen,BUN)]、炎症因子[C反应蛋白(C-reactive protein,CRP)、WBC]。结果两组手术时间、结石清除率、感染发生率比较差异无统计学意义;A组术中出血量较B组少,住院时间较B组短(P<0.05);术后1 d,A组CRP、WBC、SCr、BUN水平较B组低(P<0.05)。结论输尿管软镜碎石术与经皮肾镜碎石术治疗肾和输尿管上段结石患者,结石清除率相近,但输尿管软镜碎石术可降低患者术后炎症因子水平,减轻肾功能损伤。 Objective To explore the effects of flexible ureteroscopy lithotripsy and percutaneous nephrolithotomy on perioperative indicators and inflammatory factor levels in patients with renal and upper ureteral calculus.Methods 100 patients with renal and upper ureteral calculus who underwent surgery in the Urology department of Yingtan People's Hospital from January 2021 to December 2022 were retrospectively analyzed,they were divided into the group A and the group B according to the different surgical procedures,with 50 cases in each group.The group A underwent flexible ureteroscopy lithotripsy,while the group B underwent percutaneous nephrolithotomy.The surgical-related indicators,stone clearance rate,infection rate,renal function indicators(serum creatinine[SCr],blood urea nitrogen[BUN])and inflammatory factors(C-reactive protein[CRP],WBC)were compared between the two groups.Results There were no significant differences urgzcdl time,stone clearance rate and infection rate between the two groups;the intraoperative blood loss in the group A was less than that in the group B,and the hospital stay was shorter than that in the group B(P<0.05).1 d after surgery,the levels of CRP,WBC,SCr and BUN in the group A were lower than those in the group B(P<0.05).Conclusion Flexible ureteroscopic lithotripsy and percutaneous nephrolithotomy have similar stone clearance rates in the treatment of renal and upper ureteral calculi,but flexible ureteroscopic lithotripsy can reduce the level of postoperative inflammatory factors and reduce renal function damage.
