摘要
目的:分析妊娠期糖尿病合并妊娠期高血压疾病与新生儿结局的关系。方法:回顾性收集2019年8月-2023年7月期间于本院确诊的妊娠期糖尿病(GDM)合并妊娠期高血压疾病(HDP)患者65例作合并症组。另选取同期本院收治的GDM患者92例作GDM组;以及同期本院收治的HDP患者88例作为HDP组。分析GDM合并HDP与新生儿结局的关联性。结果:合并症组早产、低出生体重、新生儿胆红素血症发生率依次为29.23%、52.31%、12.31%,均显著高于GDM组、HDP组(P<0.05)。DM合并HDP产妇的早产危险度比GDM产妇高1.020倍(OR值=1.020,95%CI为0.567~1.836)、比HDP产妇高1.254倍(OR值=1.254,95%CI为0.799~1.967)(P<0.05)。GDM合并HDP的低出生体重危险度比GDM产妇高3.001倍(OR值=3.001,95%CI为0.869~10.365)、比HDP产妇高4.399倍(OR值=4.399,95%CI为1.157~16.726)(P<0.05)。GDM合并HDP的新生儿高胆红素血症危险度比GDM产妇高1.084倍(OR值=1.084,95%CI为0.397~2.958)、比HDP产妇高6.462倍(OR值=6.462,95%CI为2.317~18.022)(P<0.05)。结论:GDM合并HDP可对新生儿早产、新生儿高胆红素血症、低出生体重等结局风险发生协同效应,临床需预防GDM合并HDP发生。
Objective:To analyze the relationship between gestational diabetes mellitus combined with gestational hypertension and neonatal outcomes.Methods:65 patients with gestational diabetes mellitus(GDM)combined with gestational hypertension disease(HDP)diagnosed in our hospital from August 2019 to July 2023 were selected as the complication group,and 92 patients with GDM and 88 patients with HDP during the same period were selected as the HDP group.To analyze the relationship between GDM combined with HDP and neonatal outcome.Results:The rates of preterm birth,low birth weight and neonatal bilirubinemia in the complication group were 29.23%,52.31%and 12.31%,which were significantly higher than those in GDM group and HDP group(P<0.05).The risk of preterm delivery in DM women with HDP was 1.020 times higher than that in GDM women(OR=1.020,95%CI 0.567~1.836)and 1.254 times higher than that in HDP women(OR=1.254,95%CI 0.799~1.967)(P<0.05).The risk of low birth weight in women with GDM and HDP was 3.001 times higher than that in women with GDM(OR=3.001,95%CI:0.869~10.365)and 4.399 times higher than that in women with HDP(OR=4.399,95%CI:1.157~16.726)(P<0.05).The risk of neonatal hyperbilirubinemia in GDM with HDP was 1.084 times higher than that in GDM(OR=1.084,95%CI 0.397-2.958)and 6.462 times higher than that in HDP(OR=6.462,95%CI 2.317-18.022)(P<0.05).Conclusion:GDM combined with HDP may have synergistic effects on neonatal preterm birth,neonatal hyperbilirubinemia and low birth weight,and it is necessary to prevent the occurrence of GDM combined with HDP.
