摘要
目的:探讨复发性霉菌性阴道炎患者应用克霉唑阴道片联合氟康唑治疗的效果和对患者血清细胞因子水平的影响。方法:选取2022年2月至2024年4月医院收治的复发性霉菌性阴道炎患者106例,根据治疗方式不同将患者分对照组和研究组(n=53),分别口服氟康唑胶囊(首次用药剂量为每次0.4 g,次日每次0.2 g,Qd)以及在此基础上阴道置入克霉唑阴道片(1片,Qd)治疗,观察治疗有效率和症状改善时间。于治疗前和治疗21 d后采用免疫化学发光法测定肿瘤坏死因子-α(Tumor necrosis factor-alpha,TNF-α)、白细胞介素-2(Interleukine-2,IL-2)、干扰素-γ(Interferon-γ,IL-γ)、白细胞介素-4(Interleukine-4,IL-4)、白细胞介素-6(Interleukine-6,IL-6)水平,同时记录并发症发生情况和复发情况。结果:研究组治疗有效率明显高于对照组(P<0.05);研究组各项症状改善时间均明显短于对照组(P<0.05);治疗后研究组各项细胞因子水平均明显优于对照组(P<0.05);不良反应发生率两组对比无明显差异(P>0.05),复发率研究组较对照组明显更低(P<0.05)。结论:复发性霉菌性阴道炎应用氟康唑联合克霉唑阴道片治疗可改善患者症状和炎症,可提高生活质量和减少复发。
Objective:To investigate the effect of clotrimazole vaginal tablets combined with fluconazole in the treatment of recurrent fungal vaginitis patients,especially the effect on serum cytokine levels.Methods:From February 2022 to April 2024,106 patients with recurrent mycotic vaginitis admitted to the hospital were selected.Patients were divided into a control group and a study group(n=53)based on different treatment methods.The control group was treated with oral fluconazole capsules(initial dosage of 0.4 g per dose,followed by 0.2 g per dose the next day,Qd),while the study group was additionally treated with vaginal insertion of clotrimazole vaginal tablets(1 tablet,Qd)on this basis.The treatment efficacy rate and symptom improvement time were observed.Before treatment and after 21 days of treatment,the levels of tumor necrosis factor-alpha(TNF-α),interleukin-2(IL-2),interferon-gamma(IL-γ),interleukin-4(IL-4),and interleukin-6(IL-6)were measured using immunochemical luminescence assay.The occurrence of complications and recurrences were also recorded.Results:The treatment effectiveness rate of the study group was significantly higher than that of the control group(P<0.05).The improvement time of various symptoms in the research group was significantly shorter than that in the control group(P<0.05).After treatment,the levels of various cytokines in the study group were significantly better than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05),and the recurrence rate in the study group was significantly lower than that in the control group(P<0.05).Conclusion:The use of fluconazole combined with clotrimazole vaginal tablets for the treatment of recurrent fungal vaginitis can improve patients'symptoms and inflammation,enhance their quality of life,and reduce recurrence.
