摘要
文章结合现代研究以及古医籍记述,分析了十二指肠胃食管反流(DGER)的发病机制,并探讨该病发病病机及针灸辨治思路。从中医角度认为,DGER发病与中土虚寒运化不利、肝胆壅遏疏泄失常相关,以“土虚木乘”为DGER关键病机,这与现代医学研究中提出该病由上消化道运动功能障碍致病的认知相对应。在治疗上,《黄帝内经》提出了该病以针灸为主的治疗思路。针灸通过调控气机,能够恢复气机升降、补虚泄实以恢复脾胃运化、肝胆疏泄等相关脏腑器官的正常生理功能,可为DGER的临床治疗提供新的思路。
In this article,the pathogenesis of duodenal gastroesophageal reflux(DGER)is analysed in the light of modern research and ancient literature,and the pathogenesis of DGER is discussed together with the ideas of acupuncture and treatment.From the point of view of Chinese medicine,the pathogenesis of GER is related to the deficiency of middle earth and the congestion of the liver and gallbladder,and the key mechanism of GER is“the deficiency of earth and the multiplication of wood”,which corresponds to the knowledge that the disease is caused by the dysfunction of the upper gastroesophageal tract in modern medical research.In terms of treatment,the Yellow Emperor’s Classic of Internal Medicine suggests that acupuncture is the mainstay of treatment for this disease.Acupuncture can restore the normal physiological functions of the spleen and stomach,liver and gallbladder,and other related organs and tissues by regulating qi and restoring the elevation of qi and tonifying deficiency and draining solid,which may provide a new idea for the clinical treatment of DGER.
