摘要
目的:分析穴位贴敷联合中药热奄包对痰瘀互结型癌性疼痛的治疗效果。方法:选择江西中医药大学附属医院收治的80例痰瘀互结型癌性疼痛患者进行回顾性研究,纳入时间为2023年3月-2024年3月,并根据治疗方案不同分为两组。对照组(n=40)采用穴位贴敷治疗,观察组(n=40)在对照组的基础上加用中药热奄包治疗。对比两组治疗前和治疗后的中医证候积分、数字分级评分法(NRS)、匹兹堡睡眠质量指数(PSQI)、卡氏功能状态(KPS)及血清5-羟色胺(5-HT)、β-内啡肽(β-EP)水平。评估两组止痛临床疗效。结果:观察组治疗后的疼痛、胸闷脘痞、面色晦暗积分低于治疗前,且低于对照组(P<0.05)。观察组治疗后的β-EP高于对照组,5-HT低于对照组(P<0.05)。治疗后,两组PSQI、NRS评分下降,KPS评分升高,且观察组优于对照组(P<0.05)。观察组的止痛总有效率为95.00%,较对照组的75.00%高(P<0.05)。结论:穴位贴敷联合中药热奄包治疗痰瘀互结型癌性疼痛有显著的效果,可以有效提高患者的睡眠质量,减轻疼痛程度,调节血清5-HT和β-EP水平。
Objective:To analyze the efficacy of acupoint application combined with TCM hot compress package on cancer-related pain of intermin-gled phlegm and blood stasis type.Method:Eighty patients with cancer-related pain of intermin-gled phlegm and blood stasis type admitted to the Affiliated Hospital of Jiangxi University of TCM were selected for retrospective study,the inclusion period was from March 2023 to March 2024,and they were divided into two groups according to different treatment schemes.The control group(n=40)was treated with acupoint application,and the observation group(n=40)was treated with TCM hot compress package on the basis of the control group.The TCM syndromes scores,numerical rating scale(NRS),Pittsburgh sleep quality index(PSQI),Karnofsky performance status(KPS),and the levels of serum 5-hydroxytryptophan(5-HT)andβ-endorphin(β-EP)before and after treatment were compared between the two groups.The clinical efficacy of pain relief in two groups were evaluated.Result:The scores of pain,chest tightness and epigastric distension and dim complexion in the observation group after treatment were lower than those before treatment and lower than those in the control group(P<0.05).The level ofβ-EP in the observation group after treatment was higher than that in the control group,while the level of 5-HT was lower than that in the control group(P<0.05).After treatment,the scores of PSQI and NRS in the two groups decreased,the KPS scores increased,and those in the observation group were better than those in the control group(P<0.05).The total effective rate of pain relief in the observation group was 95.00%,which was higher than 75.00%in the control group(P<0.05).Conclusion:Acupoint application combined with TCM hot compress package has a significant effect in the treatment of cancer-related pain of intermin-gled phlegm and blood stasis type,it can effectively improve the sleep quality of patients,reduce the degree of pain,and regulate the levels of serum 5-HT andβ-EP.
