摘要
王海东教授将传统经筋理论与现代解剖学知识相融合,以“行气一体,阴阳同调”为核心理念,运用针刀解结术治疗早期非创伤性股骨头坏死(NONFH),形成了独具特色的诊疗体系。王教授认为早期NONFH属“骨痹”范畴,其核心病机在于“筋伤骨损,气血失调,阴阳失衡”。在治疗策略上,王教授秉承“循痛辨形,松筋调骨”的原则恢复髋关节的筋骨形态;融合“三环理论”与髋膝踝动态平衡理论,运用“循经调气,引脉通络”之法调和髋部气血;依据“阳极则反折,阴极则俯不能伸”理论,通过“调阴衡阳,正筋复和”从根本上纠正髋部屈伸失衡所反映的阴阳经筋张力失调本质。临床操作中,可采用脐下营解结术、足少阳经筋髀枢解结术及足太阳经筋通督解结术,系统性地疏解筋结、调和气血阴阳。“行气一体,阴阳同调”治疗理念下的针刀疗法不仅为NONFH的早期干预提供了有效方案,也为骨关节病的诊治开拓了思路。
Professor Wang Haidong integrates the traditional meridian-tendon theory with modern anatomical knowledge,developing a distinctive diagnostic and therapeutic system for early non-traumatic osteonecrosis of the femoral head(NONFH)based on the core philosophy of"harmonizing Qi and form,balancing Yin and Yang"through acupotomy adhesion-release.Professor Wang believes that early NONFH falls within the category of"bone Bi-disease",and its core pathogenesis lies in"tendon injury and bone damage,disorder of Qi and blood,and Yin and Yang disharmony".In terms of treatment strategies,Professor Wang adheres to the principle of"tracking pain to discern morphology,relaxing tendon to realign bones"to restore the structural integrity of the hip joint.Integrating the"triple-ring theory"with the dynamic equilibrium theory of the hip-knee-ankle chain,the method of"regulating Qi to along channel,and guiding vessel to dredging collateral"is applied to harmonize the Qi and blood in the hip.Based on the theory that"extreme Yang precipitates flexion contracture,extreme Yin induces extension disability",the fundamental imbalance of tension in the Yin and Yang meridians reflected by the hip flexion and extension imbalance is fundamentally corrected through"regulating Yin and balancing Yang,straightening tendon to restore equilibrium".In clinical practice,methods such as the umbilical lower-abdomen adhesion-release,gallbladder meridian-tendon hip-pivot adhesion-release,and the bladder meridian-tendon governor vessel-dredging adhesion-releas can be adopted to systematically unblock tendon knots and harmonize Qi,blood,Yin and Yang.The acupotomy therapy under the therapeutic concept of"harmonizing Qi and form,balancing Yin and Yang"not only provides an effective solution for the early intervention of NONFH,but also opens up new ideas for the diagnosis and treatment of osteoarthritis.
