摘要
目的:评估健脾生血颗粒联合右旋糖酐铁在妊娠期缺铁性贫血患者中的临床疗效。方法:研究于2021年1月—2024年1月进行,纳入抚州市妇幼保健院收治的100例妊娠期缺铁性贫血患者,通过随机数字表法将患者分为试验组和对照组,每组50例。对照组接受右旋糖酐铁治疗,试验组接受健脾生血颗粒联合右旋糖酐铁治疗,均持续治疗4周。对比两组症状改善状况(面色萎黄、乏力、头晕、食欲减退)、疗效、血常规[RBC、Hb、平均红细胞体积(MCV)、平均红细胞血红蛋白浓度(MCHC)]、铁代谢[血清铁蛋白(SF)、血清铁(SI)、铁调素(HEPC)、转铁蛋白(TRF)、转铁蛋白饱和度(TS)、总铁结合力(TIBC)]、不良反应(腹痛腹泻、恶心呕吐、皮疹、便秘)、不良妊娠结局(早产、胎膜早破、新生儿窒息、产后出血、产褥感染)。结果:试验组面色萎黄、乏力、头晕、食欲减退的改善时间早于对照组(P<0.05)。试验组总有效率高于对照组(P<0.05)。治疗后,试验组RBC、Hb、MCV、MCHC高于对照组(P<0.05)。治疗后,试验组TIBC、TRF低于对照组,SF、SI、HEPC、TS高于对照组(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。试验组不良妊娠结局总发生率低于对照组(P<0.05)。结论:健脾生血颗粒联合右旋糖酐铁治疗妊娠期缺铁性贫血能更有效地改善患者症状、血常规指标及铁代谢指标,减少不良妊娠结局的发生,且安全性高。
Objective:To evaluate the clinical efficacy of Jianpi Shengxue Granules combined with Iron Dextran in patients with iron deficiency anemia during pregnancy.Method:The study was conducted from January 2021 to January 2024.100 patients with iron deficiency anemia during pregnancy admitted to Fuzhou Maternal and Child Health Hospital were included.The patients were divided into the experimental group and the control group by the random number table method,with 50 cases in each group.The control group received Iron Dextran treatment,and the experimental group received Jianpi Shengxue Granules combined with Iron Dextran treatment.Both groups were treated continuously for 4 weeks.The improvement of symptoms(sallow complexion,fatigue,dizziness,reduced appetite),therapeutic effect,blood routine[RBC,Hb,mean corpuscular volume(MCV),mean corpuscular hemoglobin concentration(MCHC)],iron metabolism[serum ferritin(SF),serum iron(SI),hepcidin(HEPC),transferrin(TRF),transferrin(TS)and total iron binding capacity(TIBC)],adverse reactions(abdominal pain and diarrhea,nausea and vomiting,rash,constipation),adverse pregnancy outcomes(premature birth,premature rupture of membranes,neonatal asphyxia,postpartum hemorrhage,puerperal infection)between the two groups were compared.Result:The improvement time of sallow complexion,fatigue,dizziness and reduced appetite in the experimental group were earlier than those in the control group(P<0.05).The total effective rate of the experimental group was higher than that of the control group(P<0.05).After treatment,the RBC,Hb,MCV and MCHC in the experimental group were higher than those in the control group(P<0.05).After treatment,TIBC and TRF in the experimental group were lower than those in the control group,while SF,SI,HEPC and TS were higher than those in the control group(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).The total incidence of adverse pregnancy outcomes in the experimental group was Medical Innovation of China Vol.22,No.27 September,2025 lower than that in the control group(P<0.05).Conclusion:The combination of Jianpi Shengxue Granules and Iron Dextran in the treatment of iron deficiency anemia during pregnancy can more effectively improve the symptoms,blood routine indicators and iron metabolism indicators of patients,reduce the occurrence of adverse pregnancy outcomes,and has high safety.
