摘要
目的:通过Meta分析明确重症监护室(ICU)护士发生共情疲劳的影响因素。方法:计算机系统检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库、PubMed、Web of Science、EMbase、the Cochrane Library、Scopus等数据库中有关ICU护士共情疲劳影响因素的研究,检索时限为建库至2024年7月。由2名研究者独立筛选文献,提取资料并评价纳入研究偏倚后,采用Stata 17.0和R软件进行Meta分析。结果:共纳入20篇文献,包括5015名ICU护士,提取到35个影响因素。Meta分析结果显示:未婚、低学历、工作年限≤5年、身体状况欠佳、月值夜班数≥10次、合同护士、月收入≤6000元、未接受心理培训均是ICU护士共情疲劳的主要影响因素(P<0.05)。护理工作环境(r=-0.414,P<0.001)、社会支持(r=-0.356,P<0.001)、心理弹性(r=-0.354,P<0.001)和职业认同(r=-0.354,P=0.013)与ICU护士共情疲劳呈负相关。结论:现有证据表明,护理管理者可以根据以上影响因素及早识别ICU护士共情疲劳高风险人群,并给予个性化的干预措施,促进护理事业高质量发展。
Objective:To identify the influencing factors of compassion fatigue among intensive care unit(ICU)nurses using Meta-analysis.Methods:Computerized search was performed for studies on the influencing factors of compassion fatigue in ICU nurses in the CNKI,WanFang Data,VIP,CBM,PubMed,Web of Science,EMbase,Cochrane Library,Scopus.The retrieval time was from the inception to July 2024.Two reviewers independently screened literature,extracted data and assessed the risk of bias of the included studies.Meta-analysis was performed by using Stata 17.0 and R software.Results:A total of 20 studies,including 5015 ICU nurses and 35 influencing factors were extracted.The results of Meta-analysis showed that unmarried,low education,working life≤5 years,monthly night shifts≥10 times,contracted nurse,monthly income≤6000 yuan,and no psychological training were the main influencing factors of compassion fatigue in ICU nurses(P<0.05).Nursing work environment(r=-0.414,P<0.001),social support(r=-0.356,P<0.001),mental resilience(r=-0.354,P<0.001)and professional identity(r=-0.354,P=0.013)were negatively correlated with compassion fatigue of ICU nurses.Conclusion:Current evidence shows that according to the above factors,nursing managers can identify the high-risk groups of ICU nurses with compassion fatigue early,and give personalized intervention measures to promote the high-quality development of nursing career.
