摘要
目的:探讨经阴道彩色多普勒血流显像(CDFI)检查在子宫病变中的诊断价值,并对不同子宫病变的超声征象表现进行对比。方法:选取2022年10月至2024年10月在北京市石景山医院就诊的80例子宫病变患者,均行经阴道CDFI检查,以手术病理学检查诊断为金标准,分析经阴道CDFI检查诊断子宫病变的准确率,对比不同类型子宫病变的超声征象。结果:经阴道CDFI检查诊断子宫肌瘤的准确率为93.62%,诊断子宫腺肌瘤的准确率为96.97%,与手术病理学检查相比,差异无统计学意义(P>0.05)。经阴道CDFI检查与金标准(手术病理学检查)的诊断结果高度一致(kappa=0.898,P<0.001)。子宫肌瘤与子宫腺肌瘤患者子宫动脉舒张期血流峰值对比,差异无统计学意义(P>0.05)。与子宫腺肌瘤患者相比,子宫肌瘤患者子宫动脉搏动指数和阻力指数更低(P<0.05)。结论:经阴道CDFI检查可准确诊断子宫肌瘤和子宫腺肌瘤,两种疾病患者子宫动脉血流动力学参数存在差异,可作为疾病鉴别诊断的依据。
Objective:To explore the diagnostic value of transvaginal color Doppler flow imaging(CDFI)in uterine lesions and compare the ultrasonic features of different uterine lesions.Methods:A total of 80 patients with uterine lesions admitted to Beijing Shijingshan Hospital from October 2022 to October 2024 were selected.All patients underwent transvaginal CDFI examination.Using surgical pathological diagnosis as the gold standard,the accuracy of transvaginal CDFI in diagnosing uterine lesions was analyzed,and the ultrasonic features of different types of uterine lesions were compared.Results:The accuracy of transvaginal CDFI in diagnosing uterine fibroids was 93.62%,and that for adenomyosis was 96.97%,with no statistically significant difference compared to surgical pathological diagnosis(P>0.05).The diagnostic results of transvaginal CDFI showed high consistency with the gold standard(surgical pathology)(kappa=0.898,P<0.001).There was no significant difference in the peak diastolic blood flow velocity of the uterine artery between patients with uterine fibroids and those with adenomyosis(P>0.05).Compared to patients with adenomyosis,those with uterine fibroids had lower uterine artery pulsatility index and resistance index(P<0.05).Conclusion:Transvaginal CDFI can accurately diagnose uterine fibroids and adenomyosis.Differences in hemodynamic parameters of the uterine artery between the two diseases may serve as a basis for differential diagnosis.
