摘要
目的观察针刺联合情景互动呼吸训练治疗脑卒中后吞咽障碍的临床疗效。方法选取2022年1月-2023年6月在辽宁中医药大学附属医院康复中心治疗的脑卒中吞咽障碍患者60例,通过随机数表法将60例样本分为对照组和观察组,每组30例。对照组接受吞咽治疗、应用情景互动呼吸训练系统进行呼吸训练。观察组则在对照组的基础上,在治疗时应用针刺疗法,取舌三针、双侧风池、风府、翳风共9穴,均在吞咽治疗与呼吸训练时留针,治疗结束后将针取出,每次治疗,留针训练1次/d,5d/周,持续4周。分别于治疗前、治疗4周后,采用洼田饮水试验(Water Swallowing Test,WST)、标准吞咽功能评定量表(Standardized Swallowing Assessment,SSA)评估患者的吞咽功能;采用基于该系统进行用力肺活量(Forced Vital Capacity,FVC)、第1秒用力呼气容积(Forced Expiratory Volume inonesecond,FEV_(1))指标评估患者的呼吸功能;采用Berg平衡量表(Berg Balance Scale,BBS)评估患者的躯体平衡能力。结果2组治疗4周后吞咽功能评分相比于治疗前,WST、SSA评分均显著降低,FVC、FEV_(1)数值、Berg平衡能力评分显著升高,差异具有统计学意义(P<0.05);而相比于对照组,观察组在治疗4周后WST、SSA评分均显著降低,FVC、FEV_(1)数值、Berg平衡能力评分提高,差异具有统计学意义(P<0.05)。结论针刺联合情景互动呼吸训练可有效改善患者的吞咽功能、呼吸功能与躯体平衡能力。
Objective To observe the clinical effect of acupuncture combined with situational interactive breathing training in the treatment of dysphagia after stroke.Methods A total of 60 stroke patients with dysphagia treated in the Rehabilitation Center of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 2021 to June 2022 were selected and divided into control group and observation group by random number table method,with 30 cases in each group.The control group received the interactive breathing training system of swallowing and application.In the observation group,on the basis of the control group,acupuncture therapy was applied during treatment,and a total of 9 points of tongue three needles,bilateral Fengchi,Fengfu and Yifeng were taken.The needles were retained during swallowing treatment and breathing training,and the needles were removed after treatment.This was a single treatment,and the training with needles was once a day,5 days a week,lasting for 4 weeks.The swallowing function of the patients was assessed by WST and SSA before and 4 weeks after treatment,respectively.Forced vital capacity(FVC)and forced expiratory volume in the first second(FEV_(1))were used to evaluate the respiratory function of patients based on the system.The Berg balance Scale(BBS)was used to evaluate the patients'physical balance ability.Results After 4 weeks of treatment,WST and SSA scores of swallowing function in 2 groups were significantly lower than before treatment,while FVC,FEV1 and Berg balance scores were significantly increased,with statistical significance(P<0.05).Compared with the control group,WST and SSA scores were significantly lower in the observation group after 4 weeks of treatment,and FVC,FEV_(1)and Berg balance scores were higher,with statistical significance(P<0.05).Conclusion Acupuncture exercise combined with situational interactive respiratory training system can effectively improve the swallowing function,respiratory function and body balance ability of patients.
