太原市放射诊疗资源及其应用现状调查

Investigation of the current status of radiation diagnosis and treatment resources and their use in Taiyuan City,China

摘要 目的了解太原市辖区内放射诊疗资源及其应用现状,为优化资源配置,规范诊疗行为提供数据支持。方法采用普查的方法,使用统一的调查表对太原市放射诊疗机构基本情况进行调查;结合2023年末太原市常住人口数据计算放射诊疗资源的数量及应用频度。结果太原市共有放射诊疗机构562家,放射工作人员3970人,设备1354台,每百万人口拥有放射诊疗机构103家、放射工作人员728人,放射诊疗设备248台。放射诊疗资源区县差异显著,6城区共有放射诊疗机构526家(93.59%)、三级医院25家(96.15%),开展核医学诊疗、放射治疗的机构全部位于城区;不同级别医院相差悬殊,三级医院平均配置放射工作人员数和设备数(99.42人、17.15台)>二级医院(13.31人、4.62台)>一级及未定级医院(1.43人、1.38台)。三级医院放射诊疗频度112149人次/家,是二级医院的5倍,是一级及未定级医院的44倍。放射诊疗应用频度为975.79人次/千人口,X射线诊断频度941.12人次/千人口、介入放射学频度21.68人次/千人口、核医学诊断频度8.70人次/千人口、核医学治疗频度2.33人/千人口、放射治疗度频度1.95人/千人口。结论太原市放射诊疗资源较丰富,放射诊疗应用频度达到Ⅱ类医疗保健地区水平,仍有很大的增长和提升空间。目前放射诊疗资源和诊疗服务不同区县和不同级别诊疗机构间分布不均衡,主要集中在城区和三级医院,需要卫生行政部门统筹规划并科学配置资源。 Objective To understand the current status of radiation diagnosis and treatment resources and their use in Taiyuan City,China,and provide data support for optimizing resource allocation and standardizing diagnosis and treatment.Methods A census-based approach was implemented using a standardized questionnaire to collect basic information on radiation diagnosis and treatment institutions in Taiyuan City.The number and use frequency of radiation diagnosis and treatment resources were calculated based on the resident population of Taiyuan City at the end of 2023.Results There were a total of 562 radiation diagnosis and treatment institutions in Taiyuan City,with 3970 workers and 1354 pieces of equipment.The numbers of radiation diagnosis and treatment institutions,workers,and equipment per 1000000 population were 103,728,and 248,respectively.There were significant differences in radiation diagnosis and treatment resources among different districts and counties.There were 526(93.59%)radiation diagnosis and treatment institutions and 25(96.15%)tertiary hospitals in the six urban districts.All institutions offering nuclear medicine diagnosis and treatment and radiotherapy were located in the urban districts.There were substantial disparities among hospitals of different levels.The average numbers of radiation workers and equipment were in the order of tertiary hospitals(99.42,17.15)>secondary hospitals(13.31,4.62)>primary and unrated hospitals(1.43,1.38).The frequency of radiation diagnosis and treatment in tertiary hospitals was 112149 per hospital,which is 5 times that of secondary hospitals and 44 times that of primary and unrated hospitals.The use frequency of radiation diagnosis and treatment was 975.79 per 1000 population.The use frequency was 941.12 per 1000 population for X-ray diagnosis,21.68 per 1000 population for interventional radiology,8.70 per 1000 population for nuclear medicine diagnosis,2.33 per 1000 population for nuclear medicine treatment,and 1.95 per 1000 population for radiotherapy.Conclusion There are abundant radiation diagnosis and treatment resources in Taiyuan City.The use frequency of radiation diagnosis and treatment has reached the level of Class II medical and health care regions,and there remains considerable room for growth and improvement.At present,the distribution of radiation diagnosis and treatment resources and services is unbalanced among different districts and counties as well as among institutions of different levels,mainly concentrated in urban districts and tertiary hospitals.The health administrative department needs to make overall plans and allocate resources scientifically.
