磁共振IDEAL-IQ技术在肝硬化患者肾脏早期改变中的应用价值

摘要 目的:探讨磁共振非对称回波的最小二程估算法迭代水脂分离(IDEAL-IQ)技术评价肝硬化患者结构性肾损伤早期改变的效能。方法:选取2024年6—11月湖南省人民医院(晓庄学院附属第一医院)临床诊断为肝硬化并行上腹部磁共振检查的40例住院患者设为肝硬化组,选取同期于本院行上腹部磁共振检查的36例其他系统疾病患者设为对照组。比较两组的脂肪分数(FF)值和R_(2)^(*)值及双肾上极、中部、下极髓质的FF值与R_(2)^(*)值。结果:两组肾脏各部位髓质的FF值比较,差异无统计学意义(P>0.05)。肝硬化组左肾髓质的R_(2)^(*)值低于对照组,差异有统计学意义(P<0.05)。结论:左肾髓质R_(2)^(*)值可能是早期预测肝硬化患者结构性肾损害的最佳指标。 Objective To explore the effifficacy of the iterative decomposition of water and fat with echo asymmetry and leastsquares estimation-imaging with quantifification(IDEAL-IQ)technique based on the least two-stage estimation method of magnetic resonance echoes in evaluating the early changes of structural renal injury in patients with liver cirrhosis.Methods A total of 40 inpatients clinically diagnosed with liver cirrhosis and underwent upper abdominal magnetic resonance imaging(MRI)in Hunan Provincial People′s Hospital(the First Affiffiliated Hospital of Hunan Normal University)from June to November 2024 were selected as the liver cirrhosis group.A total of 36 patients with other systemic diseases who underwent upper abdominal MRI examination during the same period in our hospital were selected as the control group.All subjects underwent 3.0T magnetic resonance IDEAL-IQ sequence scans.The fat fraction(FF)values and R2*values,as well as the FF values and R2*values of the upper pole,middle pole and lower pole of both kidneys were compared between the two groups.Results There was no statistically signifificant difffference in the FF values of the medulla in each part of the kidneys between the two groups(P>0.05).The R2*value of the left renal medulla in the liver cirrhosis group was lower than that in the control group,and the difffference was statistically signifificant(P<0.05).Conclusions The R2*value of the left kidney medulla may be the best indicator for the early prediction of structural renal injury in patients with liver cirrhosis.
