动脉自旋标记(ASL)MR灌注成像对脑膜瘤病理分型鉴别诊断及临床应用价值

摘要 目的 探讨ASL-MR灌注成像参数(CBF、ATT)对不同病理分型脑膜瘤(WHOⅠ-Ⅲ级)的鉴别诊断价值及在手术规划中的应用意义。方法 回顾性分析2023年12月-2024年12月本院经病理证实的64例脑膜瘤患者ASL数据,比较不同分型间灌注参数差异。结果 WHO分级越高,CBF值显著增加,ATT随分级升高而缩短,表明高级别肿瘤血流更快,组间差异均有统计学意义(P<0.05);绝对CBF和ATT均可有效鉴别高级别肿瘤,联合模型AUC达0.91,敏感度为95.8%,显著优于单一指标(P<0.05)。结论 动脉自旋标记(ASL)磁共振成像技术通过标记动脉血水质子实现无创性脑血流灌注测量,能准确量化脑膜瘤的血流动力学特征,有助于制定个体化治疗方案。 Objective To explore the differential diagnostic value of ASL-MR perfusion imaging parameters(CBF,ATT)in different pathological types of meningioma(WHO gradeⅠ-Ⅲ)and its application significance in surgical planning.Methods The ASL data of 64 patients with meningioma confirmed by pathology in our hospital from December 2023 to December 2024 were analyzed retrospectively,and the differences of perfusion parameters among different types were compared.Results The higher the WHO classification,the higher the CBF value.The shortening of ATT with the increase of grade indicates that the blood flow of high-grade tumor is faster.The differences between groups were statistically significant(P<0.05).Absolute CBF and ATT can effectively differentiate high-grade tumors.The AUC of the combined model is 0.91,and the sensitivity is 95.8%,which is significantly better than that of a single index(P<0.05).Conclusion The arterial spin labeling(ASL)magnetic resonance imaging technology can realize noninvasive measurement of cerebral blood perfusion by labeling arterial blood protons,which can accurately quantify the hemodynamic characteristics of meningiomas and help to formulate individualized treatment plans.
