摘要
目的:分析给予乳腺癌保乳患者术后低分割调强放疗的效果。方法:选择2023年1月至2024年12月本院收治的78例乳腺癌保乳术治疗患者,根据术后放疗方式的不同分成对照组(n=39,常规放疗)、观察组(n=39,低分割调强放疗),对比两组的T淋巴细胞亚群水平、肿瘤标志物水平变化、放射损伤发生率以及局部复发率。结果:组间治疗前T淋巴细胞亚群、肿瘤标志物水平均无显著性差异(P>0.05);治疗后,与对照组比较,观察组的CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)水平更高,CD8^(+)水平更低(P<0.05);治疗后,观察组CEA、CA19-9、CA15-3水平均低于对照组,且两组均低于治疗前(P<0.05);观察组的放射损伤发生率较对照组低(5.13%vs 20.51%,P<0.05);组间局部复发率比较,差异无显著性(5.13%vs 10.26%,P>0.05)。结论:给予乳腺癌保乳患者术后低分割调强放疗的效果可观,对改善患者T淋巴细胞亚群、肿瘤标志物水平有积极意义,可预防或降低放射损伤风险,且不增加局部复发风险。
Objective:To analyze the effect of postoperative hypofractionated intensity-modulated radiotherapy(IMRT)in patients with breast-conserving surgery for breast cancer.Methods:A total of 78 patients who underwent breast-conserving surgery for breast cancer and were admitted to Nanjing Jiangning Hospital from January 2023 to December 2024 were selected.According to different postoperative radiotherapy methods,they were divided into a control group(n=39,conventional radiotherapy)and an observation group(n=39,hypofractionated IMRT).The levels of T lymphocyte subsets,changes in tumor marker levels,incidence of radiation injury,and local recurrence rate were compared between the two groups.Results:There were no significant differences in the levels of T lymphocyte subsets and tumor markers between the two groups before treatment(P>0.05).After treatment,compared with the control group,the levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in the observation group were higher,while the level of CD8^(+)was lower(P<0.05).After treatment,the levels of CEA,CA19-9,and CA15-3 in the observation group were lower than those in the control group,and the levels in both groups were lower than those before treatment(P<0.05).The incidence of radiation injury in the observation group was lower than that in the control group(5.13%vs 20.51%,P<0.05).There was no significant difference in the local recurrence rate between the two groups(5.13%vs 10.26%,P>0.05).Conclusion:The effect of postoperative hypofractionated IMRT in patients with breast-conserving surgery for breast cancer is considerable.It is of positive significance for improving the levels of T lymphocyte subsets and tumor markers in patients,can prevent or reduce the risk of radiation injury,and does not increase the risk of local recurrence.
