摘要
目的:探究经阴道子宫三维超声联合4K腹腔镜下子宫肌瘤剔除术的应用研究。方法:选取2024年3月—2025年1月于绵阳市人民医院妇产科接受手术治疗的70例子宫肌瘤患者,根据数字表法随机分为对照组(n=35,采用4K腹腔镜下子宫肌瘤剔除术)和研究组(n=35,采用经阴道子宫三维超声联合4K腹腔镜下子宫肌瘤剔除术)。比较两组患者的手术操作精准度、手术效果、卵巢功能和血清炎性因子水平。结果:与对照组相比,研究组手术时间更短,术中出血量更低,肌瘤完整切除率更高,术后首次下床活动时间更早(P<0.05)。术后3 d,两组卵泡刺激素(FSH)、黄体生成素(LH)水平均升高,雌二醇(E2)水平均降低,且研究组FSH、LH水平高于对照组,E2水平低于对照组(P<0.05)。术后3 d,两组肿瘤坏死因子-α(TNF-α)、白细胞介素-2(IL-2)水平均降低,且研究组低于对照组(P<0.05)。结论:经阴道子宫三维超声联合4K腹腔镜下子宫肌瘤剔除手术精准度高,对患者机体和卵巢功能损伤更小,炎性反应较轻,有利于术后恢复,值得临床推广。
Objective:To investigate the application effect of transvaginal three-dimensional(3D)uterine ultrasound combined with 4K laparoscopic myomectomy.Methods:70 uterine fibroids patients who underwent surgical treatment at the Department of Obstetrics and Gynecology,Mianyang People’s Hospital from March 2024 to January 2025 were selected.Enrolled patients were randomly divided into the control group(n=35,undergoing 4K laparoscopic myomectomy)and the study group(n=35,undergoing transvaginal 3D uterine ultrasound-guided 4K laparoscopic myomectomy)using the random number table method.Surgical precision,outcomes,ovarian function,and serum inflammatory factor levels were compared between the two groups.Results:Compared with the control group,the study group showed shorter operative time,lower intraoperative blood loss,higher complete myoma resection rate,and earlier time to first postoperative ambulation(P<0.05).On postoperative day 3,both groups showed elevated follicle stimulating hormone(FSH)and luteinizing hormone(LH)levels,along with decreased estradiol(E2)levels.The study group showed higher FSH and LH levels but lower E2 levels than the control group(P<0.05).On postoperative day 3,tumor necrosis factor-α(TNF-α)and interleukin-2(IL-2)levels decreased in both groups,with the study group showing lower levels than the control group(P<0.05).Conclusion:Transvaginal 3D uterine ultrasound-guided 4K laparoscopic myomectomy has advantages of higher precision,minimized trauma to ovarian function,milder inflammatory responses,and better postoperative recovery,which is worthy of clinical promotion.
