摘要
目的 探讨零切迹椎间融合器与椎间融合器联合钛板在颈椎前路椎间盘切除融合术(anterior cervical discectomy and fusion,ACDF)中的临床疗效及影像学差异。方法 纳入2021年1月至2024年8月本院接受ACDF的97例患者,随机分为观察组(49例)和对照组(48例)。比较两组患者手术时间、术中出血量、术后疼痛评分及术后住院天数,比较两组患者颈椎活动度、融合节段高度、椎间隙高度及颈椎曲度,分析手术前后JOA评分及NDI指数变化。结果 观察组手术时间短于对照组(P<0.05),术中出血量少于对照组(P<0.05),术后疼痛评分低于对照组(P<0.05)。两组患者术后颈椎活动度、颈椎曲度恢复、融合节段高度维持、椎间隙高度较术前改善,且观察组在颈椎活动度、颈椎曲度恢复、融合节段高度维持、椎间隙高度保持等方面优于对照组(P<0.05)。结论 零切迹椎间融合器在颈椎前路椎间盘切除融合术中表现出显著的临床和影像学优势,值得临床推广。
Objective To investigate the clinical efficacy and radiographic differences between the zero-profile interbody fusion cage and the combined interbody fusion cage with titanium plate in Anterior Cervical Discectomy and Fusion(ACDF).Methods A total of 97 patients who underwent ACDF at our hospital from January 2021 to August 2024 were included in the study.Patients were randomly divided into an observation group(49 cases)and a control group(48 cases).We compared the surgery time,intraoperative blood loss,postoperative pain scores,and length of postoperative hospitalization between the two groups.Additionally,we assessed cervical range of motion,fusion segment height,intervertebral disc height,and cervical curvature in both groups.The changes in the JOA(Japanese Orthopaedic Association)score and NDI(Neck Disability Index)were analyzed before and after surgery.Results The observation group had a shorter surgery time compared to the control group(P<0.05),less intraoperative blood loss(P<0.05),and lower postoperative pain scores(P<0.05).Both groups showed improvements in cervical range of motion,cervical curvature recovery,fusion segment height maintenance,and intervertebral disc height postoperatively compared to preoperative values.The observation group outperformed the control group in cervical range of motion,cervical curvature recovery,fusion segment height maintenance,and intervertebral disc height retention(P<0.05).Conclusion The zero-profile interbody fusion cage demonstrates significant clinical and radiographic advantages in ACDF surgery and is worthy of clinical promotion.
