摘要
[目的]分析2008—2020年江苏省连云港市结直肠癌发病趋势及年龄变化特征。[方法]收集整理2008—2020年覆盖连云港市所有县区结直肠癌数据资料,质控后计算结直肠癌各年份粗发病率、中国人口标化发病率、平均发病年龄、中标平均发病年龄和中标年龄发病构成比等指标。应用Joinpoint回归模型分析(中标)发病率的平均年度变化百分比(average annual percentage change,AAPC),使用线性回归模型计算(中标)平均发病年龄与年份之间的相关性,计算并比较其2008年和2020年中标年龄发病构成比变化。[结果]2008—2020年连云港市结直肠癌中标发病率呈明显上升趋势,AAPC为3.68%(P<0.001)。全市结直肠癌的中标平均发病年龄由2008年62.22岁增长至2020年的63.03岁,年均增长β=0.132岁(P=0.013)。各年龄组发病率趋势中,50岁以下各组无增长趋势,50~59岁组开始出现增长趋势,60岁及以上各组均呈现明显增长趋势,60~69岁组发病率增长幅度最大。[结论]2008—2020年连云港市结直肠癌发病水平呈现明显上升趋势,尤其是50岁及以上年龄组人群。针对目前流行情况,建议采用适宜的多重防控措施,降低结直肠癌疾病负担。
[Objective]To analyze the trends of incidence and onset age of colorectal cancer(CRC)in Lianyungang City,Jiangsu Province from 2008 to 2020.[Methods]The data of CRC were collected from cancer registries in Lianyungang City from 2008 to 2020.The crude incidence rate,age-stan-dardized incidence rate by Chinese standard population(ASIRC),average age at onset,the stan-dardized average age at onset by Chinese population and standardized age-specific incidence com-position by Chinese population of colorectal cancer were calculated.The average annual percentage change(AAPC)in the crude incidence rate and ASIRC was analyzed by the Joinpoint regression model.The correlation between the average age at onset and the year was analyzed by Linear re-gression model.The standardized age-specific incidence composition was calculated and compared between 2008 and 2020.[Results]The ASIRC of CRC in Lianyungang City showed upward trends from 2008 to 2020 with an AAPC was 3.68(P<0.001).The standardized mean age at onset in-creased from 62.22 years old in 2008 to 63.03 years old in 2020 with an average annual growthβof 0.132 years(P=0.013).From 2008 to 2020,the incidence of colorectal cancer bagan to increase from the age group of 50~59 years old,showing a significant increasing trend after age of 60 years old with a marked increase in the age group of 60~69 years old.[Conclusion]The incidence of CRC showed a significant upward trend in Lianyungang City from 2008 to 2020,especially in the age group of 50 years old and above.In response to the current epidemic situation,it is recommended to adopt appropriate multiple prevention and control measures to reduce the burden of CRC.
