摘要
早发性卵巢功能不全是严重影响女性生殖健康、身心健康的妇科难治病。高慧教授认为早发性卵巢功能不全发生与肾-天癸-冲任-胞宫轴失调密切相关,肾脾两虚、冲任受损为基本病机,肝郁血瘀、胞脉受阻贯穿整个病程。临证以补肾调冲为原则,同时配合活血化瘀、疏肝理气、行气祛痰诸法,寓通于补,以补兼通;结合月经周期气血阴阳盛衰规律,卵泡期重在补养肝肾,精血充盈方可滋养胞宫与卵泡;排卵期重在活血化瘀以助卵子排出;黄体期宜健脾益气以促进黄体形成;行经期活血通经以助月经下行通畅,整个治疗过程辅以生活方式调理与情志调摄,形神兼备。综合疗法不仅针对核心病机,还有助于调和脏腑功能,为临床治疗早发性卵巢功能不全提供策略。
Premature ovarian insufficiency is a refractory gynecological disease that seriously affects female reproductive health,physical and mental health.Professor Gao Hui believes that the occurrence of premature ovarian insufficiency is closely related to the disharmony of the kidney-Tiangui-Chongren-uterus axis.The basic pathogenesis is the spleen and kidney deficiency and the damage to the Chongren meridian.Stagnation of liver qi and blood stasis and uterine vessels block run through the whole course of the disease.The clinical treatment adopts the principle of tonifying kidney and regulating Chong meridian,and at the same time,methods of promoting blood circulation for removing blood stasis,dispersing liver and regulating qi,and promoting qi and eliminating phlegm are combined.The principle of combining tonification with promoting circulation is applied.According to the law of the waxing and waning of qi,blood,yin and yang during the menstrual cycle,the follicular phase is focused on nourishing the liver and kidney,and full essence and blood can nourishing the uterus and follicles.During the ovulation period,the focus is on promoting blood circulation for removing blood stasis to facilitate the release of the egg.During the luteal stage,the emphasis is on invigorating spleen and replenishing qi to promote the formation of the corpus luteum.During the menstrual phase,promoting blood circulation for regulating menstruation helps the menstruation to flow smoothly.The whole treatment process is supplemented by lifestyle regulation and emotional adjustment,which is both physical and mental.This comprehensive therapy not only targets the core pathogenesis,but also harmonizes the functions of the Zangfu,providing a strategy for the clinical treatment of premature ovarian insufficiency.
