下一代测序技术在15例鹦鹉热衣原体肺炎诊治中的临床分析

Clinical analysis of diagnosis and treatment in 15 patients with Chlamydia psittaci pneumonia by next generation sequencing

摘要 目的探讨下一代测序技术在鹦鹉热衣原体肺炎诊治中的临床特征及治疗价值。方法收集2022年1月6日至2024年3月5日于景德镇市第一人民医院、景德镇市第二人民医院通过下一代测序技术诊断为鹦鹉热衣原体肺炎的15例患者的临床资料,分析患者一般资料、临床表现、基础疾病、实验室检查、影像学特点、气管镜检查结果、下一代测序结果、治疗和预后情况。结果15例患者中,13例普通患者采用靶向下一代测序技术,2例重症患者采用宏基因组学测序;男4例,女11例,年龄34~70岁;临床症状主要为发热、咳嗽、咳痰、胸闷等。实验室检查血气分析显示,10例血氧分压下降,中性粒细胞计数正常,淋巴细胞计数偏低,C反应蛋白、乳酸脱氢酶、D-二聚体均升高。影像学特征为斑片状密度增高影、支气管充气征、磨玻璃影、胸腔积液,支气管检查提示气道炎症改变。治疗以多西环素、喹诺酮类药物为主,患者均好转出院,无死亡病例。结论下一代靶向测序技术有助于明确诊断鹦鹉热衣原体肺炎,可使患者得到及时有效的治疗,降低其死亡率,值得临床推广。 Objective To explore the clinical features and therapeutic value of next-generation sequencing in Chlamydia psittaci pneumonia.Methods The clinical data of 15 patients diagnosed with Chlamydia psittaci pneumonia via next generation sequencing technology at the First People's Hospital of Jingdezhen City and the Second People's Hospital of Jingdezhen City from January 6,2022 to March 5,2024 were collected,the patient's general data,clinical manifestations,underlying diseases,laboratory tests,imaging character-istics,bronchoscopy results,next-generation sequencing results,treatment and prognosis were analyzed.Results There were 15 patients in this study,13 common patients were sequenced by targeted next generation sequencing,2 severe patients were sequenced by metagenomics next generation sequencing;4 males and 11 females,aged from 34 to 70 years old.The main clinical symptoms were fever,cough,expectoration,chest tightness and so on.Laboratory tests and blood gas analysis showed the partial pressure of oxygen in the blood of 10 patients decreased,the neutrophil count was normal,the lymphocyte count was low,and C-reactive protein,lactate dehydrogenase and D-dimer were all elevated.The imaging features of Chlamydia psittaci pneumonia were patchy increased density shadows,bronchial sign,ground glass shadow,pleural effusion,and bronchial examination showed airway inflammation.The patients were mainly treated with doxycycline and quinolones,and all the patients were discharged,without death.Conclusion The next generation targeted sequencing technique is helpful for the definite diagnosis of Chlamydia psittaci pneumonia,which can make the patients get timely and effective treatment and reduce their mortality,which is worthy of clinical promotion.
