摘要
比较磁共振胰胆管造影(MRCP)与经内镜逆行胰胆管造影(ERCP)在70岁以上老年胆道结石诊断中的效能。选取2023年3月—2025年1月贵州医科大学第二附属医院收治的70岁以上老年患者50例,均因临床疑似胆道结石而行ERCP检查及治疗,所有患者均同时接受MRCP和ERCP检查。以手术病理结果为金标准,比较两种检查方式的诊断效能及图像质量。结果显示,ERCP与MRCP的准确度、特异度、灵敏度差异均无统计学意义(P>0.05);两种方法在图像清晰度、分辨率、对比度及细节可见度方面的评分差异均无统计学意义(P>0.05)。研究发现,MRCP与ERCP在70岁以上老年胆道结石诊断中效能相当,图像质量无显著差异。MRCP具有无创、无需造影剂、安全性高的优势,更易被老年患者接受;ERCP则兼具诊断与治疗功能,可同步进行内镜下取石。
To compare the diagnostic efficacy of magnetic resonance cholangiopancreatography(MRCP)and endoscopic retrograde cholangiopancreatography(ERCP)in elderly patients aged over 70 with biliary tract stones.A total of 50 elderly patients aged over 70 admitted to The Second Affiliated Hospital of Guizhou Medical University from March 2023 to January 2025 were selected.All patients underwent ERCP for diagnosis and treatment due to clinical suspicion of biliary tract stones,and all simultaneously received MRCP and ERCP examinations.Using surgical pathological results as the gold standard,the diagnostic efficacy and image quality of the two examination methods were compared.The results showed that there were no statistically significant differences in accuracy,specificity,and sensitivity between ERCP and MRCP(P>0.05);there were also no statistically significant differences in the scores of image clarity,resolution,contrast,and detail visibility between the two methods(P>0.05).The study concluded that MRCP and ERCP have comparable diagnostic efficacy in elderly patients aged over 70 with biliary tract stones,and there is no significant difference in image quality.MRCP has the advantages of being noninvasive,no need for contrast agents,and high safety,making it more acceptable to elderly patients;ERCP,on the other hand,has both diagnostic and therapeutic functions and can perform endoscopic stone extraction simultaneously.
