摘要
目的:观察血清抗磷脂酶A2受体(PLA2R)抗体表达与原发性膜性肾病(PMN)病情的相关性和预后评估价值。方法:选取2022年1月—2024年12月淮安市第二人民医院收治的96例PMN患者,根据抗PLA2R抗体表达情况分为阳性组(n=61)与阴性组(n=35)。比较两组临床资料,分析血清抗PLA2R抗体表达与PMN患者病情特征的关系;比较不同预后患者的抗PLA2R抗体滴度。结果:阳性组分期为Ⅳ期比例、尿素氮(BUN)及24 h尿蛋白定量水平高于阴性组(P<0.05)。Spearman相关性分析显示,PMN患者血清抗PLA2R抗体表达水平与临床分期、BUN及24h尿蛋白定量水平呈正相关(P<0.05)。治疗后,缓解组血清抗PLA2R抗体滴度水平较治疗前降低,且低于未缓解组(P<0.05)。受试者工作特征曲线显示,抗PLA2R抗体滴度评估患者预后的最佳截断值为640.90 RU/mL,曲线下面积为0.816,敏感度与特异度分别为63.40%、90.91%。结论:血清抗PLA2R抗体表达情况与PMN患者病情密切相关,其滴度水平有助于评估临床预后。
Objective:To observe the correlation between the expression of serum antiphospholipase A2 receptor(PLA2R)anti⁃body and the condition of primary membranous nephropathy(PMN),and its prognostic evaluation value.Methods:A total of ninetysix PMN patients admitted to Huai'an Second People's Hospital from January 2022 to December 2024 were selected and divided into the positive group(n=61)and the negative group(n=35)according to the expression of anti-PLA2R antibodies.The clinical data of the two groups were compared to analyze the relationship between the expression of serum anti-PLA2R antibody and the dis⁃ease characteristics of PMN patients.The titers of anti-PLA2R antibodies in patients with different prognoses were compared.Re⁃sults:The proportion of stageⅣcases,blood urea nitrogen(BUN)and 24-hour urine protein quantification levels in the positive group were higher than those in the negative group(P<0.05).Spearman correlation analysis showed that the expression level of se⁃rum anti-PLA2R antibody in PMN patients was positively correlated with clinical stage,BUN and 24-hour urine protein quantifica⁃tion(P<0.05).After treatment,the serum anti-PLA2R antibody titer level in the remission group was lower than that before treat⁃ment and was also lower than that in the non-remission group(P<0.05).The receiver operating characteristic curve showed that the optimal cutoff value of anti-PLA2R antibody titer for evaluating the prognosis of patients was 640.90 RU/mL,with an area under the curve of 0.816,and the sensitivity and specificity were 63.40%and 90.91%.Conclusion:The expression of serum anti-PLA2R antibody is closely related to the condition of PMN patients,and its titer level is helpful for evaluating the clinical prognosis.
