摘要
目的 探讨清中汤联合质子泵抑制剂治疗急性胃溃疡出血的临床疗效及其对炎症因子的影响。方法选择2023年3月至2025年2月石阡县中医医院收治的52例急性胃溃疡出血患者,依据随机数字表法,分为观察组(清中汤联合质子泵抑制剂,n=26)和对照组(质子泵抑制剂,n=26)。比较两组患者的临床治疗效果、幽门螺杆菌(Hp)根除情况、炎症指标、预后情况、胃黏膜保护功能。结果 观察组患者的临床治疗总有效率(96.15%vs. 73.08%)和Hp根除率(92.31%vs. 69.23%)明显高于对照组(P<0.05)。治疗前,两组患者炎症因子C反应蛋白(CRP)、白介素-6(IL-6)、白介素-8(IL-8)、肿瘤坏死因子α(TNF-α)和胃黏膜保护功能相关指标(胃蛋白酶原Ⅰ、胃蛋白酶原Ⅱ)水平相比无明显差异(P>0.05)。治疗后,两组患者以上炎症因子和胃蛋白酶原Ⅱ水平均明显下降,胃蛋白酶原Ⅰ水平明显升高(P<0.05);其中,观察组患者CRP、TNF-α、IL-6、IL-8、胃蛋白酶原Ⅱ水平降幅更明显,胃蛋白酶原Ⅰ水平升幅更明显(P<0.05)。观察组患者呕血改善时间、黑便改善时间和住院时间均短于对照组(P<0.05),转外科手术率和再出血率低于对照组(P<0.05)。结论 急性胃溃疡出血患者的治疗中,清中汤联合质子泵抑制剂的治疗方案可有效清除Hp,降低机体炎症水平,提升胃黏膜的保护功能,改善患者预后,临床疗效明显。
Objective To evaluate the clinical efficacy of Qing Zhong Decoction combined with proton pump inhibitors(PPIs)in the treatment of acute gastric ulcer bleeding and its effects on inflammatory factors.Methods A total of 52 patients with acute gastric ulcer bleeding,who were admitted to Shiqian County Traditional Chinese Medicine Hospital between March 2023 and February 2025,were selected.Patients were randomly divided into two groups using a random number table:the observation group(Qing Zhong Decoction combined with PPIs,n=26)and the control group(PPIs alone,n=26).The clinical therapeutic eff ects,Helicobacter pylori(Hp)eradication rates,inflammatory markers,prognosis,and gastric mucosal protection were compared between the two groups.Results The total eff ective clinical treatment rate(96.15%vs.73.08%)and Hp eradication rate(92.31%vs.69.23%)in the observation group were significantly higher than those in the control group(P<0.05).Before treatment,there were no significant diff erences between the two groups in the levels of inflammatory factors,including C-reactive protein(CRP),interleukin-6(IL-6),interleukin-8(IL-8),tumor necrosis factor-alpha(TNF-α),and gastric mucosal protection-related indicators(pepsinogen I and pepsinogen II)(P>0.05).After treatment,the levels of these inflammatory factors and pepsinogen II significantly decreased,while pepsinogen I levels significantly increased in both groups(P<0.05).Notably,the observation group showed a more significant reduction in CRP,TNF-α,IL-6,IL-8,and pepsinogen II levels,as well as a more significant increase in pepsinogen I levels(P<0.05).The time for improvement of hematemesis,melena,and the length of hospital stay were shorter in the observation group compared to the control group(P<0.05).Additionally,the observation group had lower rates of surgical conversion and rebleeding than the control group(P<0.05).Conclusion The treatment regimen of Qing Zhong Decoction combined with proton pump inhibitors eff ectively eradicates Hp,reduces inflammation,enhances gastric mucosal protection,and improves patient prognosis in the treatment of acute gastric ulcer bleeding.The clinical efficacy is significant.
