摘要
目的了解河南省健康人群甲型肝炎病毒(Hepatitis A antibody,HAV)IgG抗体水平。方法采用多阶段随机抽样方法于2021-2023年每年在河南省选取1~79岁健康人群开展问卷调查和血清标本采集;采用酶联免疫吸附试验定性检测血清HAV IgG抗体,分析抗体阳性率。结果2021年、2022年、2023年调查对象HAV IgG抗体总阳性率分别为77.33%(6889/8909)、82.09%(8063/9822)、87.04%(9683/11125);各年龄组抗体阳性率分别在63.49%~97.27%(χ^(2)=893.84,P<0.001)、70.26%~97.33%(χ^(2)=808.10,P<0.001)、81.16%~98.33%(χ^(2)=355.86,P<0.001)之间,其中1~19岁vs 20~79岁分别为66.71%vs 91.18%、73.80%vs 94.86%、82.51%vs 93.81%。在1~14岁儿童中,有vs无/不详甲型肝炎疫苗(Hepatitis A vaccine,HepA)免疫史抗体阳性率分别为64.19%vs61.89%(χ^(2)=0.92,P=0.336)、72.44%vs 72.11%(χ^(2)=0.03,P=0.864)、82.95%vs 75.04%(χ^(2)=22.92,P<0.001)。结论2021-2023年河南省健康人群HAV IgG抗体水平呈逐年升高趋势,HAV自然隐性感染水平较高;需加强儿童HepA常规接种和成人甲型肝炎防控工作。
Objective To explore hepatitis A virus(HAV)IgG antibody levels among healthy individuals in Henan province.Methods We used a multi-stage random sampling method to recruit 1-79-year-olds in Henan in 2021,2022,and 2023 for questionnaire-based surveys;we obtained serum samples from survey participants.We used enzyme-linked immunosorbent assay to qualitatively test sera for HAV IgG antibody to determine positivity rates.Results Overll positivity rates were 77.33%(6889/8909),82.09%(8063/9822),and 87.04%(9683/11125)among subjects in 2021,2022,and 2023,respectively,with 63.49%to 97.27%(χ^(2)=893.84,P<0.001),70.26%to 97.33%(χ^(2)=808.10,P<0.001),and 81.16%to 98.33%(χ^(2)=355.86,P<0.001)positive for all age groups;positivity rates for 1-19-us 20-79-year-olds were 66.71%vs 91.18%,73.80%vs 94.86%,and 82.51%us 93.81%.Among 1-14-year-old children,positivity rates of children who had received hepatitis A vaccine(HepA)us those who had not received HepA or whose vaccination history was unknown were 64.19%us 61.89%(χ^(2)=0.92,P=0.336),72.44%vs 72.11%(χ^(2)=0.03,P=0.864),and 82.95%vs 75.04%(χ^(2)=22.92,P<0.001)in 2021,2022,and 2023.Conclusions HAV IgG antibody levels increased over time among healthy individuals 1-79 years of age in Henan during 2021-2023;there was a high level of natural HAV infection.We should strengthen routine HepA vaccination for children and hepatitis A prevention and control for adults.
