摘要
目的探讨雷火灸协同中药热奄包穴位外敷疗法,在防治湿热下注型混合痔术后并发尿潴留患者中的临床应用价值。方法前瞻性选取2024年4月-2025年4月我院收治的湿热下注型混合痔术后尿潴留患者160例,按随机数表法分为对照组与观察组,每组各80例。对照组采用术后常规护理,观察组在对照组基础上增加雷火灸联合中药热奄包。比较两组临床疗效、排尿情况、不良反应发生情况。结果观察组临床总有效率高于对照组(P<0.05)。观察组首次排尿时间短于对照组,首次排尿量和术后2h内排尿量高于对照组(均P<0.05)。观察组首次排尿通畅程度高于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论雷火灸联合中药热奄包穴位外敷疗法应用于湿热下注型混合痔术后尿潴留患者,对其排尿功能及中医临床症状有较好的改善作用,值得临床推广应用。
Objective To explore the intervention effect and safety of Leihuo moxibustion combined with external application of hot compress packs to acupoints on postoperative urinary retention of damp-heat type mixed hemorrhoids.Methods A total of 160 patients with postoperative urinary retention after damp-heat descending mixed hemorrhoids admitted to our hospital from April 2024 to April 2025 were prospectively selected and divided into the control group and the observation group according to the random number table method,with 80 cases in each group.The control group received postoperative routine care and induction of urination measures,while the observation group,on the basis of the control group,added lightning and fire moxibustion combined with Traditional Chinese Medicine hot compress packs.The clinical efficacy,urination conditions and occurrence of adverse reactions of the two groups were compared.Results The total clinical effective rate of the observation group was higher than that of the control group(P<0.05).The time of the first urination in the observation group was shorter than that in the control group,while the volume of the first urination and the volume of urination within 2 hours after the operation were higher than those in the control group(all P<0.05).The patency of the first urination in the observation group was higher than that in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination of thunder and fire moxibustion and external application of traditional Chinese medicine hot compress packs on acupoints has a good improvement effect on urinary retention after surgery for mixed hemorrhoids of damp-heat descending type,as well as on their urinary function and clinical symptoms in traditional Chinese medicine.It is worthy of clinical promotion and application.
