摘要
目的探讨补肺活血胶囊联合抗结核治疗对肺结核伴糖尿病患者的临床疗效。方法选取2021年7月至2023年6月上饶市第二人民医院收治的120例初治肺结核合并糖尿病患者为研究对象,按治疗方法不同为对照组和研究组,每组60例。对照组接受2HRZE[异烟肼(isoniazid,H);利福平(rifampin,R);吡嗪酰胺(pyrazinamide,Z);乙胺丁醇(ethambutol,E)]/4HRE方案+常规降糖药物治疗,研究组在对照组基础上加用补肺活血胶囊治疗。比较两组临床症状改善情况、治疗总有效率、血糖指标、痰转阴率、CT病灶吸收情况、空洞减少情况、免疫功能、肝功能和凝血功能。结果研究组乏力发生率为45.00%(27/60)、盗汗发生率为11.67%(7/60),均低于对照组65.00%(39/60)、26.67%(16/60),差异均有统计学意义(χ^(2)=4.848、4.454,P均<0.05)。但两组发热、咳嗽、咯血发生率比较,差异均无统计学意义(P均>0.05)。两组痰转阴率、肺部病灶吸收及空洞闭合、减少患者数相比,差异均无统计学意义(P均>0.05)。研究组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,研究组餐后2 h血糖、糖化血红蛋白、白细胞介素-6、CD4^(+)水平均优于对照组,差异均有统计学意义(P均<0.05)。治疗后,两组空腹血糖、淋巴细胞、活化部分凝血活酶时间、谷丙转氨酶水平比较,差异均无统计学意义(P均>0.05)。结论补肺活血胶囊联合2HRZE/4HRE方案治疗肺结核合并糖尿病患者,可以改善患者盗汗、乏力的症状,减轻机体炎性反应。
Objective To investigate the clinical efficacy of Bufei Huoxue Capsule combined with anti-tuberculosis treatment in patients with pulmonary tuberculosis and diabetes mellitus.Methods A total of 120 patients with newly diagnosed pulmonary tuberculosis complicated with diabetes mellitus admitted to the Sec⁃ond People's Hospital of Shangrao City from July 2021 to June 2023 were selected as the research objects.Ac⁃cording to different treatment methods,they were divided into control group and study group,with 60 cases in each group.The control group received 2HRZE[isoniazid(H),rifampin(R),pyrazinamide(Z),ethambutol(E)]/4HRE regimen+conventional hypoglycemic drugs,the study group was treated with Bufei Huoxue Capsule on the basis of the control group.The improvement of clinical symptoms,total effective rate of treatment,blood glu⁃cose indexes,sputum negative conversion rate,CT lesion absorption,cavity reduction,immune function,liver function and coagulation function were compared between the two groups.Results The incidence of fatigue in the study group was 45.00%(27/60),and the incidence of night sweats was 11.67%(7/60),which were lower than 65.00%(39/60)and 26.67%(16/60)in the control group,the differences were statistically significant(χ^(2)=4.848,4.454,both P<0.05).However,there was no significant difference in the incidence of fever,cough and he⁃moptysis between the two groups(all P>0.05).There was no significant difference in sputum negative conver⁃sion rate,lung lesion absorption,cavity closure and reduction in the number of patients between the two groups(all P>0.05).The total effective rate of treatment in the study group was higher than that in the control group,the difference was statistically significant(P<0.05).After treatment,the levels of 2-hour postprandial blood glu⁃cose,glycated hemoglobin A1c,interleukin-6 and CD4^(+) in the study group were better than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,there were no significant differences in the levels of fasting blood glucose,lymphocytes,activated partial thromboplastin time and alanine aminotransferase between the two groups(all P>0.05).Conclusion Bufei Huoxue Capsule combined with 2HRZE/4HRE regimen in the treatment of patients with pulmonary tuberculosis complicated with diabetes mel⁃litus can improve the symptoms of night sweats and fatigue,and reduce the body's inflammatory response.
