摘要
目的观察温通法中药复方治疗冲任失调证乳腺增生病的临床疗效。方法本研究采用随机对照试验方法,将79例冲任失调证乳腺增生病患者随机分为2组,治疗组(40例,予温通方口服)与对照组(39例,予他莫昔芬口服),疗程为2个月经周期。比较两组治疗前后乳房疼痛积分、肿块积分、中医证候积分、超声影像学参数(腺体厚度、导管内径)、阻力指数(RI)及血清雌二醇(E2)、孕激素(P)、垂体泌乳素(PRL)水平变化,统计临床有效率与不良反应。结果治疗后,治疗组总有效率显著高于对照组(χ^(2)=8.351,P=0.039);患者疼痛积分、肿块积分、中医证候积分均低于对照组(P<0.001);治疗组患者乳腺超声各参数降低幅度均优于对照组(P<0.05),且腺体厚度变化与肿块积分降低呈正相关(P<0.001)、RI值与疼痛积分呈正相关(P=0.001);治疗组患者P水平显著升高(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论温通法可有效缓解冲任失调证乳腺增生病患者临床症状,调节内分泌水平。
Objective To observe the clinical efficacy of Wentong(warming and dredging)therapy with a compound traditional Chinese medicine formula in the treatment of fibrocystic breast disease with syndrome of disharmony of Chong Ren meridians.Methods This study used a randomized controlled trial design.A total of 79 patients with fibrocystic breast disease and syndrome of disharmony of Chong Ren meridians were randomly divided into two groups:the treatment group(40 cases,treated with Wentong formula orally)and the control group(39 cases,treated with Tamoxifen orally).The treatment duration was two menstrual cycles.The changes in breast pain scores,lump scores,TCM syndrome scores,ultrasound parameters(gland thickness,duct diameter),resistance index(RI),and serum levels of estradiol(E2),progesterone(P),and prolactin(PRL)were compared before and after treatment.Clinical effectiveness and adverse reactions were also evaluated.Results After treatment,the total effective rate in the treatment group was significantly higher than that in the control group(χ^(2)=8.351,P=0.039).The pain score,lump score,and TCM syndrome score in the treatment group were all lower than those in the control group(P<0.001).The reduction in ultrasound parameters in the treatment group was better than that in the control group(P<0.05).A positive correlation was found between changes in gland thickness and the reduction in lump score(P<0.001),and between RI values and pain scores(P=0.001).The level of P in the treatment group increased significantly(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Wentong therapy can effectively relieve clinical symptoms in patients with fibrocystic breast disease with syndrome of disharmony of Chong Ren meridians and regulate endocrine levels.
