摘要
目的:基于慢性病轨迹模式构建活动期类风湿关节炎(RA)衰弱病人护理管理方案。方法:以慢性病轨迹模式为理论指导,整合量性研究、质性研究、文献研究结果,通过小组会议逐步细化完善形成活动期RA衰弱病人护理管理方案初稿,开展2轮德尔菲专家函询,进一步修订形成方案终稿。结果:2轮专家函询问卷有效回收率均为100.00%。专家权威系数为0.94。第2轮专家函询急性期、亚急性期的条目重要性得分分别为4.48~5.00分和4.43~5.00分;肯德尔和谐系数分别为0.236和0.217(均P<0.001);变异系数分别为0.00~0.11和0.00~0.12。第2轮专家函询急性期、亚急性期的条目可行性得分分别为4.35~5.00分和4.00~4.96分;肯德尔和谐系数分别为0.192和0.207(均P<0.001);变异系数分别为0.00~0.13和0.04~0.20。最终构建的活动期RA衰弱病人护理管理方案包含急性期一级条目3个、二级条目7个、三级条目25个,亚急性期一级条目3个、二级条目7个、三级条目29个。结论:本研究构建的活动期RA衰弱病人护理管理方案具有科学性、可靠性、全面性、临床适用性与可行性。
Objective:To construct a nursing management protocol for frail protocol with active rheumatoid arthritis(RA)based on the chronic illness trajectory model.Methods:Guided by the chronic illness trajectory model,quantitative research,qualitative research,and literature review findings were integrated.The initial draft of the nursing management protocol for frail patients with active RA was gradually refined and improved through panel discussions.Two rounds of Delphi expert consultations were conducted to further revise and finalize the protocol.Results:The effective response rates for both rounds of expert consultations were 100.00%.The expert authority coefficient was 0.94.In the second round of consultations,the items importance scores for the acute phase and subacute phase ranged from 4.48 to 5.00 and 4.43 to 5.00,respectively.The Kendall's harmony coefficients were 0.236 and 0.217,respectively(all P<0.001).The variation coefficients ranged from 0.00 to 0.11 and 0.00 to 0.12,respectively.In the second round of consultations,the items feasibility scores for the acute phase and subacute phase ranged from 4.35 to 5.00 and 4.00 to 4.96,respectively.The Kendall's harmony coefficients were 0.192 and 0.207,respectively(all P<0.001).The variation coefficients ranged from 0.00 to 0.13 and 0.04 to 0.20,respectively.The final protocol included 3 first-level items,7 second-level items,and 25 third-level items for the acute phase,and 3 first-level items,7 second-level items,and 29 third-level items for the subacute phase.Conclusions:The the nursing management protocol for frail patients with active RA constructed in this study was scientific,reliable,comprehensive,clinically applicable,and feasible.
