摘要
目的:收集并评估慢性肾脏病非透析病人营养治疗相关指南,总结指南推荐意见。方法:系统检索国内外网站及数据库中关于慢性肾脏病非透析病人营养治疗的相关指南,检索时限为2010年1月1日—2024年3月24日。采用临床指南研究与评价工具对纳入指南进行评价,采用内容分析法提取指南推荐意见并进行汇总。结果:最终纳入14部指南,其中12部为A级推荐,1部为B级推荐,1部为C级推荐;推荐意见主要包括蛋白质、热量、钠、钾、磷、钙6个方面,汇总的推荐意见共39条。不同指南对慢性肾脏病病人营养治疗的推荐意见总体相似,部分领域存在差异。结论:慢性肾脏病非透析病人营养治疗相关指南高质量证据不足,建议多开展慢性肾脏病病人营养治疗方面的高质量原始研究,构建高质量慢性肾脏病病人营养治疗循证指南。
Objective:To collect and to evaluate guidelines on nutritional therapy for non-dialysis patients with chronic kidney disease,and to summarize the recommendations.Methods:Guidelines on nutritional therapy for non-dialysis patients with chronic kidney disease were retrieved from Chinese and international databases and websites.The retrieval time spanned from January 1,2010 to March 24,2024.The Appraisal of Guidelines for Research&EvaluationⅡwas used to assess the included guidelines.The content analysis was applied to extract and to summarize the recommendations.Results:A total of 14 guidelines were included,of which 12 were rated as grade A,1 as grade B,and 1 as grade C.The recommendations primarily focused on six aspects,namely protein,calories,sodium,potassium,phosphorus,and calcium,with a total of 39 summarized recommendations.Overall,the recommendations across different guidelines were similar,though some variations existed in specific areas.Conclusions:There was a lack of high-quality evidence in current guidelines on nutritional therapy for non-dialysis patients with chronic kidney disease.It was recommended to conduct more high-quality original research on nutritional therapy for patients with chronic kidney disease and to develop evidence-based guidelines of higher quality.
