摘要
目的:探究皮瓣修复术在手部骨折伴皮肤软组织损伤中的运用效果。方法:回顾性分析我院于2019年7月-2021年7月收治的66例手部骨折伴皮肤软组织缺损患者,根据皮瓣修复术式分为对照组和观察组,各33例,对照组予以掌背皮瓣移植,观察组予以指背筋膜皮瓣修复移植。观察两组围手术期资料和手部功能、并发症、供区愈合及疼痛情况。结果:观察组术中出血量低于对照组(P<0.05)、骨折愈合时间、骨折外固定时间和住院天数短于对照组(P<0.05);手部功能优良率和供区良好愈合率高于对照组,并发症发生率和术后10 d、2 m的VAS评分低于对照组(P<0.05)。结论:皮瓣修复术在手部骨折伴皮肤软组织缺损的临床效果良好,指背筋膜皮瓣修复移植可减少手术损伤,促进患者术后愈合,缩短住院时长,改善手部功能,降低并发症发生率和术后疼痛。
Objective:To explore the application effect of skin flap repair in hand fracture with skin and soft tissue injury.Methods:A retrospective analysis of 66 patients with hand fractures and skin and soft tissue defects admitted to our hospital from July 2019 to July 2021 was divided into control group and observation group according to the flap repair method,33 cases in each group.The control group was treated with dorsal metacarpal flap transplantation,and the observation group was treated with dorsal fascial flap repair and transplantation.The perioperative data,hand function,complications,donor site healing and pain of the two groups were observed.Results:The intraoperative blood loss in the observation group was lower than that in the control group(P<0.05),and the fracture healing time,fracture external fixation time and hospitalization days were shorter than those in the control group(P<0.05).The excellent and good rate of hand function and the good healing rate of donor area were higher than those of the control group,and the incidence of complications and VAS scores at 10 days and 2 months after operation were lower than those of the control group(P<0.05).Conclusions:Flap repair has a good clinical effect in hand fracture with skin and soft tissue defect.Dorsal fasciocutaneous flap transplantation can reduce surgical injury,promote postoperative healing,shorten hospitalization time,improve hand function,and reduce the incidence of complications and postoperative pain.
