摘要
目的探讨自牵引后离断三角吻合BillrothⅠ式消化道重建在全腹腔镜远端胃癌根治术中的应用价值。方法回顾性分析2021年6月至2024年6月于徐州医科大学附属淮安医院胃肠外科接受全腹腔镜远端胃切除术、BillrothⅠ式消化道重建患者的临床资料,其中三角吻合组25例,Overlap组35例,比较两组术中相关指标及术后的疗效。结果两组均完成腹腔镜手术,无中转开放手术。三角吻合组在术中手术时间、消化道吻合时间以及术后首次排气时间上短于Overlap组(P<0.05),差异有统计学意义;两组在术中出血量、淋巴结清扫数量、术后进食时间、住院时间以及并发症发生率上比较,差异无统计学意义(P>0.05)。结论全腹腔镜下远端胃癌根治术中采取自牵引后离断三角吻合消化道重建是一种安全有效的手术方式,可简化手术操作,减少术中吻合时间,有助于患者术后恢复。
Objective To investigate the application value of BillrothⅠdigestive tract reconstruction by self-pulling and latter transection followed by delta-shaped anastomosis in total laparoscopic distal gastrectomy.Methods Retrospective analysis was performed on the clinical data of patients who underwent total laparoscopic distal gastrectomy and BillrothⅠdigstive tract reconstruction at Huai'an Hospital of Xuzhou Medical University from June 2021 to June 2024.The delta-shaped anastomosis group consisted of 25 patients and Overlap group had 35 patients.A comparison was made between the two groups in terms of intraoperative and postoperative efficacy indexes.Results The laparoscopic surgery was successfully completed in both groups without conversion to open surgery.In comparison to Overlap group,the delta-shaped anastomosis group exhibited significantly shorter operation time,digestive tract anastomosis time,and postoperative first flatus time(P<0.05).There was no significant difference between the two groups in terms of intraoperative blood loss,time to oral intake,lymph node dissection number,postoperative hospital stay,and complication rates(P>0.05).Conclusions It can be concluded that total laparoscopic distal gastrectomy with self-pulling and latter transec-tion followed by delta-shaped anastomosis is a safe and effective surgical technique for reconnecting the digestive tract.This approach can simplify the procedure,reduce intraoperative anastomosis time,and facilitate early postoperative rehabilitation for patients.
