乳果糖、布拉氏酵母菌散配合饮食调节治疗儿童功能性便秘的临床疗效及对肠道菌群的影响

Clinical Efficacy of Lactulose and Saccharomyces Boulardii Powder Combined with Diet Regulation in Treatment of Functional Constipation in Children and Its Influence on Intestinal Flora

摘要 目的:探讨乳果糖、布拉氏酵母菌散配合饮食调节治疗儿童功能性便秘(FC)的临床疗效,以及对肠道菌群的影响。方法:将2023年1月至2024年9月于我院就诊的60例≥3岁且病程≥3个月的FC患儿随机分为观察组和对照组,各30例,观察组予乳果糖联合布拉氏酵母菌散治疗,对照组予乳果糖治疗,除此之外,2组均配合饮食调节,均连续治疗1个月。统计2组临床总体疗效,治疗前后便秘症状改善情况,主要肠道菌群(乳杆菌、双歧杆菌、肠杆菌、葡萄球菌和酵母菌)含量变化,治疗后3个月复发情况,并进行对比。结果:1)临床疗效及便秘症状:观察组治疗总有效率(100%,30/30)高于对照组(86.7%,26/30),P<0.05。与治疗前相比,治疗后2组患儿排便困难发生情况、粪便性状、每次排便时间、排便频次、腹胀发生情况评分均明显降低(P<0.05);治疗后2组间比较,观察组患儿各便秘症状评分均明显低于对照组(P<0.05)。2)肠道菌群变化:与治疗前相比,治疗后2组患儿肠道双歧杆菌含量明显升高(P<0.05),乳杆菌、肠杆菌、葡萄球菌、酵母菌含量明显降低(P<0.05);治疗后2组间比较,观察组患儿肠道双歧杆菌含量更高(P<0.05),乳杆菌、肠杆菌、葡萄球菌、酵母菌含量更低(P<0.05)。3)复发情况:观察组复发率(6.7%,2/30)低于对照组(19.2%,5/26),P<0.05。结论:乳果糖、布拉氏酵母菌散配合饮食调节治疗儿童FC,可有效调节肠道菌群,提高临床疗效,降低复发率。 Objective To explore the clinical efficacy of lactulose and Saccharomyces boulardii powder com⁃bined with diet regulation in the treatment of functional constipation(FC)in children and its influence on intesti⁃nal flora.Methods A total of 60 children with FC aged≥3 years and with a disease course of≥3 months who visited our hospital from January 2023 to September 2024 were randomly divided into an observation group and a control group,with 30 cases in each group.The observation group was treated with lactulose combined with Saccharomyces boulardii powder,while the control group was treated with lactulose alone.In addition,both groups were given diet regulation,and the treatment lasted for 1 month continuously.The overall clinical efficacy of the two groups,the improvement of constipation symptoms before and after treatment,the changes in the contents of main intestinal flora(Lactobacillus,Bifidobacterium,Enterobacter,Staphylococcus and Sac⁃charomyces),and the recurrence situation 3 months after treatment were statistically analyzed and compared.Results 1)Clinical efficacy and constipation symptoms:The total effective rate of the observation group(100%,30/30)was higher than that of the control group(86.7%,26/30)(P<0.05).Compared with before treatment,the scores of defecation difficulty,fecal properties,time of each defecation,defecation frequency and abdominal distension occurrence in the two groups of children decreased significantly after treatment(P<0.05).After treatment,the scores of various constipation symptoms in the observation group were significantly lower than those in the control group(P<0.05).2)Changes in intestinal flora:Compared with before treat⁃ment,the content of Bifidobacterium in the intestinal tract of the two groups of children increased significantly after treatment(P<0.05),and the contents of Lactobacillus,Enterobacter,Staphylococcus and Saccharomy⁃ces decreased significantly(P<0.05).After treatment,the content of Bifidobacterium in the intestinal tract of the observation group was higher than that in the control group( P<0. 05), and the contents of Lactobacillus, Enterobacter, Staphylococcus and Saccharomyces were lower than those in the control group( P<0. 05). 3) Recurrence situation: The recurrence rate of the observation group( 6. 7%, 2/30) was lower than that of the control group( 19. 2%, 5/26)( P<0. 05). Conclusion Lactulose and Saccharomyces boulardii powder combined with diet regulation in the treatment of FC in children can effectively regulate the intestinal flora, improve the clinical efficacy and reduce the recurrence rate.
