摘要
目的 提升驻科药师医嘱审核质量。方法 分析驻科药师开展医嘱审核中存在的问题,据此,以规范医嘱审核行为为切入点,以药师的工作流程为主线,并以医院呼吸科为例,探索如何通过流程优化提升驻科药师医嘱审核水平,总结整改成效。结果 通过与医护流程融合、优化审方模式等改进措施,驻科药师医嘱审核工作流程明显改善。2024年上半年,该院驻科药师审核医嘱数较2023年同期增加17.00%;发现问题医嘱531条,给予用药建议545份,医师采纳率96.78%。引入临床药师工作站实现工作记录电子化后,驻科药师每日缩短人工记录时间2 h,并助力科室实现抗菌药物使用强度达标。结论 驻科药师对医嘱的审核是其开展临床药学服务的基础。本研究中通过优化驻科药师的医嘱审核工作流程,使用药监护过程更全面及深入,进一步提高了医院药学服务质量。
Objective To improve the quality of medical order review by resident pharmacists.Methods Problems in the medical order review process conducted by resident pharmacists were analyzed.Then,approaches to enhance the medical order review proficiency of resident pharmacists through process optimization and to summarize improvement outcomes were explored by standardizing medical order review practices as the entry point,using the pharmacists′workflow serving as the main focus,taking the respiratory department as an example.Results The medical order review workflow by resident pharmacists was significantly improved through improvement measures of integration with medical and nursing processes and optimization of the prescription review model.In the first half of 2024,the number of medical orders reviewed by resident pharmacists increased by 17.00%compared with the same period in 2023,531 problematic medical orders were identified,545 medication recommendations were provided and the rate of physician adoption was 96.78%.Resident pharmacists′daily manual record time reduced by 2 h and contributed to antibacterial drug use intensity in the department to standard,after the pharmacist workstation introduced and the digitalization of work records.Conclusion Medical order review by resident pharmacists is the basis for their clinical pharmaceutical services.In this study,the optimization of the medical order review workflow makes the medication monitoring process more comprehensive and in-depth,thereby enhancing the quality of pharmaceutical services in the hospital.
