摘要
目的探讨基于健康管理模式的延续性护理在老年高血压合并冠心病经皮冠状动脉介入术(PCI)后患者中的应用效果。方法回顾性选取2023年1月至12月邹平市人民医院心内科收治的98例老年高血压合并冠心病PCI术后患者作为研究对象,按照入院时间分为对照组(47例)与观察组(51例),对照组采用常规随访,观察组采用基于健康管理模式的延续性护理。比较两组患者随访前后的服药依从性、情绪状态、自我效能感、血压控制情况及生活质量。结果观察组随访后的服药依从性、自我效能感、生活质量、血压达标率高于对照组,汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)评分均低于对照组,差异有统计学意义(P<0.05)。结论基于健康管理模式的延续性护理对老年高血压合并冠心病PCI术后患者有益,能增强服药依从性,减轻负面情绪,提高自我效能感、血压控制率及生活质量。
Objective To explore the application effect of continuous nursing based on health management model on elderly patients with hypertension complicated with coronary heart disease after percutaneous coronary intervention(PCI).Methods A total of 98 elderly patients with hypertension complicated with coronary heart disease who underwent PCI admitted to the Department of Cardiology of Zouping People's Hospital from January to December 2023 were retrospectively selected as the research objects,and they were divided into the control group(47 cases)and the observation group(51 cases)according to the admission time.The control group received routine follow-up,while the observation group received continuous nursing based on the health management model.The medication compliance,emotional state,self-efficacy,blood pressure control and quality of life of the two groups of patients before and after follow-up were compared.Results The medication compliance,self-efficacy,quality of life and blood pressure compliance rate of the observation group after follow-up were higher than those of the control group,while the Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)scores were lower than those of the control group,the differences were statistically significant(P<0.05).Conclusion The continuous nursing based on health management model is beneficial for elderly patients with hypertension complicated with coronary heart disease after PCI.It can enhance medication compliance,alleviate negative emotions,improve self-efficacy,blood pressure control rate and quality of life.
