摘要
目的探讨经皮肝穿胆道置管引流(PTCD)置换术后精细化护理在胆管癌患者胆道引流管理中的应用价值。方法选取2023年5月—2025年1月在本院接受PTCD置换术的60例胆管癌患者为研究对象,随机将其分为对照组与实验组,每组30例。对照组采用常规化护理模式,实验组在对照组基础上额外接受精细化护理模式,监测并记录两组患者的引流管留置时长、胆道引流通畅度评分、生活质量评分及并发症发生率。结果实验组患者的引流管留置时间明显短于对照组,且胆道引流通畅性评分显著高于对照组(P<0.05);护理后,两组标准化的生活质量量表(EORTC QLQ-C30)评分均较护理前升高,且实验组优于对照组(P<0.05);实验组患者的并发症发生率明显低于对照组(P<0.05)。结论在PTCD置换术后进行精细化护理,可有效提高胆管癌患者的胆道引流水平,提升患者生命质量,降低并发症发生率,具备显著的临床应用潜力,应积极地在临床中推广实施。
Objective To investigate the application value of refined nursing in the management of biliary drainage in patients with cholangiocarcinoma after PTCD replacement.Methods In this study,60 patients with cholangiocarcinoma who underwent PTCD replacement from May 2023 to January 2025 were randomly divided into two groups:control group and experimental group,with 30 cases in each group.The control group received routine nursing mode,and the experimental group received additional refined nursing mode on the basis of the control group.The drainage tube indwelling time,biliary drainage patency score,quality of life score and complication rate of the two groups of patients were monitored and recorded.Results The retention time of the drainage tube in the experimental group was significantly shorter than that in the control group,and the patency score of biliary drainage was significantly higher than that in the control group(P<0.05);After nursing,the standardized quality of life scale(EORTC QLQ-C30)scores of both groups increased compared to before nursing,and the experimental group was better than the control group(P<0.05);The incidence of complications in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion Fine nursing care after PTCD replacement surgery can effectively improve the drainage level of bile ducts in patients with cholangiocarcinoma,enhance their quality of life,reduce the incidence of complications,and has significant clinical application potential.It should be actively promoted and implemented in clinical practice.
