摘要
目的探讨改良H型海绵体位垫在中重度急性呼吸窘迫综合征(ARDS)患者俯卧位通气治疗中的作用。方法以2023年6月—2024年5月本院收治的40例中重度ARDS病患为研究对象,将其设为实验组,该组患者在改良H型海绵体位垫的支持下接受俯卧位通气治疗。将同期收治的40例仅接受俯卧位通气的中重度ARDS病患设为对照组。对两组病患的干预前后数据进行对比分析,以及分析压力性损伤的发生率以及血气分析结果和血流动力学的变化情况。结果干预后,两组PaO_(2)、PaCO_(2)、PaO_(2)/FiO_(2)等血气分析水平均有所改善,实验组优于对照组(P<0.05);两组HR干预前后差异均不明显(P>0.05);MAP、CVP水平干预后均比干预前有所升高,实验组优于对照组(P<0.05);对照组发生压力性损伤的发生率高于实验组(P<0.05);干预后实验组腹腔压力明显低于对照组(P<0.05)。结论在中重度ARDS患者采用俯卧位通气治疗的过程中,应用改良H型海绵体位垫可显著降低压力性损伤的风险,并优化血气分析指标及血流动力学参数,具有显著的临床优势,值得进一步推广应用。
Objective To explore the role of improved H-type cavernous position pad in prone ventilation treatment in patients with moderate to severe acute respiratory distress syndrome(ARDS).Methods The 40 moderate to severe ARDS patients enrolled between June 2023 and May 2024 were the research subjects,and they were set as experimental group.The patients in this group received prone ventilation with the support of a modified H-type cavernous position cushion.At the same time,the control group consisted of 40 patients with moderate to severe ARDS who received only prone ventilation during the same period.The data before and after intervention of the two groups of patients were compared and analyzed to analyze the incidence of stress injury,blood gas analysis results and hemodynamic changes.Results After the intervention,the blood gas analysis level of PaO_(2),PaCO_(2),PaO_(2)/FiO_(2) and other groups improved,and the experimental group was better than the control group(P<0.05);the difference between the two groups was not obvious before and after the HR intervention(P>0.05);the levels of MAP and CVP were higher than before and before the intervention,and the experimental group was better than the control group(P<0.05);the probability of pressure injury in the control group was 20.00%,while the experimental group was 7.50%,which was obvious(P<0.05);the abdominal pressure in the experimental group was significantly lower than that in the control group,and the difference was obvious(P<0.05).Conclusion In the process of prone ventilation treatment in patients with moderate to severe ARDS,the application of modified H-type cavern position pads can significantly reduce the risk of pressure injury and optimize blood gas analysis indicators and hemodynamic parameters.It has significant clinical advantages and deserves further promotion and application.
