摘要
围手术期持续性的疼痛不仅会给肛肠科患者带来严重的心理负担,还会严重影响患者术后生活质量,致使患者延迟出院,创面愈合时间也会延长。目前,如何减轻肛肠科患者术后疼痛程度、提升患者生活质量以及加速其快速康复,是目前肛肠科加速康复外科的研究重点。中西医结合加速康复外科模式在肛肠科围手术期疼痛管理实施具有显著临床效果。在术前,中医特色宣教及中医情志调理能够改善患者负性情绪,提高治疗的依从性;在术中,针药复合麻醉、穴位刺激、针药平衡麻醉能够减少麻药用量,减轻手术应激反应;在术后,中西医结合多模式镇痛管理,能够提高肛肠科患者围手术期镇痛效果,加速术后快速康复。本文主要围绕肛肠科围手术期疼痛管理,对当前中西医结合加速康复模式的最新进展与特色优势进行综合阐述。
Persistent pain during the perioperative period not only imposes a severe psychological burden on patients in the Department of Anorectal,but also seriously affects their postoperative quality of life,leading to delayed discharge and prolonged wound healing time.At present,how to reduce the postoperative pain level of patients in the Department of Anorectal,improve their quality of life and accelerate their rapid recovery is the current research focus of enhanced recovery after surgery in the Department of Anorectal.The integrated traditional Chinese and western medicine enhanced recovery after surgery model has a significant clinical effect in the implementation of perioperative pain management in the Department of Anorectal.Before the operation,traditional Chinese medicine characteristic education and traditional Chinese medicine emotional regulation can improve patients'negative emotions and enhance treatment compliance.During the operation,combined acupuncture and drug anesthesia,acupoint stimulation,and balanced acupuncture and drug anesthesia can reduce the dosage of anesthetics and alleviate surgical stress responses.After the operation,the multi-modal analgesic management combining traditional Chinese and western medicine can improve the perioperative analgesic effect of patients in the anorectal department and accelerate the rapid recovery after the operation.This article mainly focuses on perioperative pain management in the Department of Anorectal and comprehensively elaborates on the latest progress and distinctive advantages of the current integrated traditional Chinese and western medicine enhanced recovery model.
