摘要
目的探讨Nd:YAG激光联合Er:YAG激光行闭合瓣牙冠延长术的疗效及优势。方法选取2019年1月至2023年8月苏州口腔医院收治的40例(40颗患牙)因冠折就诊患者作为研究对象,按照随机数字表法分为对照组和试验组,每组20例。对照组行传统牙冠延长术,试验组行Nd:YAG激光与Er:YAG激光辅助下的闭合瓣牙冠延长术,比较两组患者术后的疼痛程度及龈缘位置。结果两组患者术后1、2、3 d VAS评分均低于本组术后3 h,术后2、3 d VAS评分均低于本组术后1 d,且术后3 d VAS评分低于术后2 d,差异有统计学意义(P<0.05)。试验组术后3 h、术后1、2 d VAS评分均低于对照组,差异有统计学意义(P<0.05)。术后3 d,两组患者的VAS评分比较,差异无统计学意义(P>0.05)。对照组术后2、3个月患牙的龈缘位置均高于术后1个月,差异有统计学意义(P<0.05)。术后1个月,试验组患牙的龈缘位置高于对照组,差异有统计学意义(P<0.05)。术后2、3个月,两组患牙的龈缘位置比较,差异无统计学意义(P>0.05)。结论Nd:YAG激光联合Er:YAG激光进行闭合瓣牙冠延长术临床疗效良好,有助于减少手术创伤,减轻患者术后不适,促进组织愈合。
Objective To investigate the efficacy and advantages of Nd:YAG laser combined with Er:YAG laser for closed flap crown lengthening.Methods A total of 40 patients(40 affected teeth)admitted to Suzhou Stomatological Hospital for crown fracture from January 2019 to August 2023 were selected as the study subjects,and they were divided into the control group and the experimental group according to the random number table method,with 20 cases in each group.The control group underwent traditional crown lengthening,and the experimental group underwent closed flap crown lengthening assisted by Nd:YAG laser and Er:YAG laser.The postoperative pain level and gingival margin position were compared between the two groups.Results The VAS scores of patients postoperative 1,2,3 d in both groups were lower than those of our group in the 3 h postoperative period,and the VAS scores of patients in the 2 and 3 d postoperative period were lower than those of our group in the 1 d postoperative period,and the VAS scores of patients in the 3 d postoperative period was lower than that of the 2 d postoperative period,and the differences were statistically significant(P<0.05).The VAS scores of the experimental group were lower than those of the control group at 3 h postoperatively,1 and 2 d postoperatively,and the differences were statistically significant(P<0.05).At 3 d postoperatively,the VAS scores of the two groups of patients were compared,and the difference was not statistically significant(P>0.05).The gingival margin position of the affected teeth at 2 and 3 months postoperatively in the control group were higher than those at 1 month postoperatively,and the differences were statistically significant(P<0.05).At 1 month postoperatively,the gingival margin position of the affected teeth in the test group was higher than that of the control group,and the difference was statistically significant(P<0.05).At 2 and 3 months postoperatively,the gingival margin positions of the affected teeth in the two groups were compared,and the differences were not statistically significant(P>0.05).Conclusion Nd:YAG laser combined with Er:YAG laser for closed flap crown lengthening is clinically effective,helping to reduce surgical trauma,alleviate postoperative discomfort in patients,and promote tissue healing.
