摘要
目的探讨非重症专业低年资护士病情观察管理行为的真实体验,为制定提升策略提供参考依据。方法根据计划行为理论框架制定访谈提纲,采用现象学研究方法对武汉市某三甲医院的12名非重症专业低年资护士进行半结构式访谈,并运用Colaizzi 7步分析法提炼主题。结果基于计划行为理论框架中行为态度、主观规范、知觉行为控制的3个维度,共提炼出8个主题:积极态度、消极态度、团体压力、责任压力、患方压力、个人能力不足、工作负荷较大、外部资源支持不足。结论多数受访者对于病情观察管理持积极态度,病情观察管理行为意向受到同事领导、岗位职责、患方及家属的影响,同时还存在个人能力不足、工作负荷较大及外部资源支持不到位的阻碍因素。医院及护理管理者应充分调动护士的主观能动性,提供更多的培训机会及学习平台,制定科学、系统的培训策略,提升护士的病情观察能力。
Objective To investigate the real-life experiences of junior nurses in non-critical care specialties regarding the observation and management of various conditions of patients,in or⁃der to provide a reference point for developing enhancement strategies.Methods The interview outline was formulated according to the theoretical framework of planned behavior,a semi-struc⁃tured interview was conducted in 12 nurses from non-critical care specialty department of a tertiary grade A hospital in Wuhan,and the data were analyzed by Colaizzi seven-step analysis method.Results Based on the theory of planned behavior,8 themes were extracted,including positive at⁃titude,negative attitude,group pressure,responsibility pressure,patient pressure,personal inca⁃pacity,high workload,and insufficient support from external resources.Conclusions Most of the respondents held a positive attitude towards disease observation and management,but they are influenced by colleagues'leadership,job responsibilities,patients and families in the process of condition observation.There are also hindering factors such as lack of personal ability,large workload and support from external resources.Hospitals and nursing managers should fully mobi⁃lize the subjective initiative of nurses,provide more training opportunities and learning platforms,and formulate scientific and systematic training strategies to enhance nurses'ability in condition observation and management.
