摘要
中药药物警戒是药物警戒工作的主要内容之一。已经明确的中药药物警戒基本上包括了毒性警戒、配伍禁忌、妊娠禁忌、饮食禁忌、量效警戒、不良反应警戒、中药养护、炮制、煎煮、服用以及中成药生产、运输、储存、使用等方面的警戒,但缺失了中药遴选环节中体现出的警戒思维。本文以二仙汤为例,对其方证辨证和组方潜药进行深入探析,尤其重视二仙汤组方潜药时运用的中药遴选思维,通过对知“方药法象模式下中药遴选”之常而达“二仙汤实际组方潜药”之变的研究,认为药物警戒的概念内涵应该向前拓展至中医合理辨证基础上组方潜药时的中药遴选环节,以补充最符合中医“前瞻性预防理念”的中药警戒思维,并以此建立更为完整的“中医药临床用药警戒思维导图”。
Pharmacovigilance of traditional Chinese medicine is one of the main contents of pharmacovigilance work.The pharmacovigilance of traditional Chinese medicine that has been clearly defined basically includes toxicity vigilance,incompatibility contraindication,pregnancy contraindication,diet contraindication,dose-effect vigilance,adverse reaction vigilance,traditional Chinese medicine maintenance,processing,decocting,taking and Chinese patent medicine production,transportation,storage,use and other aspects of vigilance,but it lacks the vigilance thinking reflected in the selection of traditional Chinese medicine.Taking Erxian decoction as an example,this paper makes an in-depth analysis of its prescription syndrome differentiation and prescription latent drugs,especially paying attention to the selection thinking of traditional Chinese medicine used in the prescription latent drugs of Erxian decoction.Through the study of mastering the routine of"selecting Chinese medicine under the model of prescription and method"to achieve the adaptation of"Erxian decoction actual prescription and latent medicine",it is believed that the concept of pharmacovigilance should be extended to the selection of traditional Chinese medicine when composing latent drugs on the basis of reasonable syndrome differentiation of traditional Chinese medicine,so as to supplement the traditional Chinese medicine vigilance thinking that is most in line with the"prospective prevention concept"of traditional Chinese medicine,and to establish a more complete"Chinese medicine clinical medication vigilance mind map".
