摘要
目的点评临床上质子泵抑制剂(PPIs)注射剂的不合理使用情况并分析不合理的原因,积极采取相应的改进措施,促进医院PPIs的规范化用药。方法通过徐州矿务集团总医院的瑞美合理用药系统和信息管理系统,抽调出2024年1月至3月应用PPIs注射剂的住院患者信息,根据患者临床诊断、病情程度、肝肾功能检验指标及用药史等,分析、评价PPIs注射剂使用的合理性。结果徐州矿务集团总医院2024年1月至3月使用PPIs注射剂患者共2363例,其中使用注射用艾司奥美拉唑1321例,使用注射用奥美拉唑804例,使用注射用艾普拉唑166例,使用注射用泮托拉唑64例,使用注射用雷贝拉唑8例;消化内科、放疗科、普通外科为此次PPIs注射剂点评医嘱条数较多的科室。PPIs注射剂不合理使用55例(2.33%),包括适应证不适宜24例(43.64%),药物遴选不适宜7例(12.73%),用法用量不适宜19例(34.55%),联合用药不适宜3例(5.45%),重复给药2例(3.64%)。结论医院使用PPIs注射剂存在适应证、药物遴选、用法用量、联合用药以及重复给药等多种不合理情况。临床药师应致力于推动医院出台多项举措,进一步提高PPIs合理使用水平。
Objective To review the irrational use of proton pump inhibitors(PPIs)injection in clinic and analyze the causes of irrational use,and actively take corresponding improvement measures to promote the standardized use of PPIs in hospitals.Methods The information of inpatients who used PPIs injection from January to March 2024 was extracted from the Ruimei rational drug use system and information management system of Xuzhou Mining Group General Hospital.The rationality of PPIs injection was analyzed and evaluated according to the clinical diagnosis,severity,liver and kidney function test indexes and medication history of patients.Results A total of 2363 patients were treated with PPIs injection in Xuzhou Mining Group General Hospital from January to March 2024,including 1321 cases of esomeprazole for injection,804 cases of omeprazole for injection,166 cases of ilaprazole for injection,64 cases of pantoprazole for injection and 8 cases of rabeprazole for injection;gastroenterology,radiotherapy,and general surgery were the departments with a large number of medical orders for this PPIs injection review.There were 55 cases(2.33%)of irrational use of PPIs injection,including 24 cases(43.64%)of unsuitable indications,7 cases(12.73%)of unsuitable drug selection,19 cases(34.55%)of unsuitable usage and dosage,3 cases(5.45%)of unsuitable combined medication,and 2 cases(3.64%)of repeated administration.Conclusion There are many unreasonable situations in the use of PPIs injection in hospitals,such as indications,drug selection,usage and dosage,combined medication,repeated administration and so on.Clinical pharmacists should be committed to promoting the hospital to introduce a number of initiatives to further improve the level of rational use of PPIs.
