摘要
目的:分析CT和MR在超急性期脑梗死诊断中的价值。方法:选取影像科2021年4月~2024年4月接收的疑似超急性期脑梗死患者120例为观察对象,予以CT诊断、MR诊断,采用手术或溶栓治疗,即刻开展血管造影,得出最终诊断结果,分析CT诊断、MR诊断结果,计算诊断特异度、灵敏度、阳性预测值及准确度。结果:120例患者经CT诊断检出超急性期脑梗死患者82例,经MR诊断检出超急性期脑梗死患者90例,但组间差异不明显(P>0.05)。以最终治疗结果为金标准,CT组诊断超急性期脑梗死真阳性74例,真阴性12例,MR组诊断超急性期脑梗死真阳性85例,真阴性15例;MR组诊断准确度高于CT组(P<0.05),特异度、灵敏度、阳性预测值略高于CT组,但组间差异不明显(P>0.05)。结论:CT、MR两种影像学技术在超急性期脑梗死诊断中均具有一定参考价值,MR诊断特异度及灵敏度更佳,诊断价值略优于CT诊断。
Objective:To analyze the value of CT and MR in the diagnosis of hyperacute cerebral infarction.Methods:120 suspected hyperacute cerebral infarction patients admitted to the imaging department from April 2021 to April 2024 were selected as the observation subjects,and CT and MR diagnoses were given.Surgery or thrombolysis treatment was adopted,and immediate angiography was performed to obtain the final diagnostic result.CT and MR diagnostic results were analyzed,and diagnostic specificity,sensitivity,positive predictive value,and accuracy were calculated.Results:Among the 120 patients,82 were diagnosed with hyperacute cerebral infarction through CT,and 90 were diagnosed with hyperacute cerebral infarction through MR,with no significant difference between the two groups(P>0.05).Based on the final treatment result as the gold standard,there were 74 cases of true positive and 12 cases of true negative diagnosis in the CT group for hyperacute phase cerebral infarction,and 85 cases of true positive and 15 cases of true negative diagnosis in the MR group for hyperacute phase cerebral infarction.The accuracy of the MR group were higher than those of the CT group(P<0.05),and the diagnostic specificity,sensitivity,and positive predictive value was slightly higher than that of the CT group,but the difference between the two groups was not significant(P>0.05).Conclusion:Both CT and MR imaging techniques have certain reference value in the diagnosis of hyperacute cerebral infarction.MR diagnosis has better specificity and sensitivity,and its diagnostic value is slightly better than CT diagnosis.
