2023-2024年甘肃部分地市儿童急性呼吸道感染中病毒流行情况和人腺病毒基因组序列特征分析

Analysis of the Prevalence and Genome Sequence Characteristics of Human Adenovirus in Children with Acute Respiratory Infections in Selected Cities of Gansu Province,China from 2023-2024

摘要 研究2023-2024年甘肃省兰州市、天水市儿童急性呼吸道感染中人腺病毒(Human adenovirus,HAdV)感染的流行病学特征和基因组序列进化特征,为儿童呼吸道感染性疾病的防控提供基础数据和科学依据。2023年1月1日-2024年1月1日在兰州市、天水市2家哨点医院收集690份儿童急性呼吸道感染标本,使用多重呼吸道病原检测试剂进行检测,使用Hep-2细胞对HAdV阳性标本进行病毒分离。用IBM SPSS 26.0软件对数据进行统计分析,对成功分离的HAdV毒株进行全基因组测序,对获得全基因组序列进行基因组序列特征分析。690份儿童急性呼吸道感染病例咽拭子标本中HAdV阳性检出率为10.87%(75/690),不同性别之间HAdV的检出率差异无统计学意义(P>0.05);HAdV检出率6月~组最高(20.41%)、1岁~组最低(5.11%),各年龄组HAdV检出率差异有统计学意义(χ^(2)=11.054,P=0.026);HAdV秋季检出率最高(16.23%),春季最低(4.44%),不同季节患儿HAdV检出率差异有统计学意义(χ^(2)=16.376,P=0.001)。基因组序列进化分析显示,病毒分离株(TS070342)属于HAdV-C5,病毒分离株(LZ09350)、(LZ10023)、(LZ10026)属于HAdV-B3,病毒分离株(TS070342)由HAdV-C5、HAdV-C89组成的重组病毒。HAdV是2023-2024年除流感病毒、新型冠状病毒以外引起儿童急性呼吸道感染的主要病毒,具有明显的年龄和季节特征,病毒分离株(TS070342)基因组内包含型内重组事件,提示应加强HAdV分子流行特征监测。 This study investigates the epidemiological characteristics and genome sequence evolution of human adenovirus(HAdV)infections in children with acute respiratory tract infections in Lanzhou and Tianshui,Gansu Province,China from 2023–2024.The findings aim to provide foundational data and scientific insights for the prevention and control of respiratory infections in children.Between January 1,2023,and January 1,2024,690 specimens were collected from children with acute respiratory infections at two sentinel hospitals in Lanzhou and Tianshui,China.Multiplex respiratory pathogen detection reagents were employed for pathogen identification,and Hep-2 cells were used for virus isolation of HAdV-positive specimens.Statistical analyses were performed using IBM SPSS 26.0 software.Whole-genome sequencing was conducted on successfully isolated adenovirus strains,and bioinformatics tools were utilized to analyze genome sequence characteristics.The detection rate of HAdV was 10.87%(75/690)in throat swab samples,with no significant difference observed between genders(P>0.05).The highest detection rate(20.41%)was found in the 6-month-old group,while the lowest rate(5.11%)was observed in the 1-year-old group,showing statistically significant differences among age groups(χ^(2)=11.054,P=0.026).Seasonally,the detection rate was highest in autumn(16.23%)and lowest in spring(4.44%),with statistically significant differences across seasons(χ^(2)=16.376,P=0.001).Phylogenetic analysis revealed that the virus isolate TS070342 belonged to HAdV-C5,while isolates LZ09350,LZ10023,and LZ10026 belonged to HAdV-B3.Notably,isolate TS070342 was identified as a recombinant virus comprising HAdV-C5 and HAdV-C89.HAdV was found to be a primary causative agent of acute respiratory infections in children from 2023 to 2024,apart from influenza virus and SARS-CoV-2,demonstrating distinct age and seasonal characteristics.The intra-type recombination event identified in isolate TS070342 underscores the necessity of strengthening molecular epidemiological monitoring of HAdV.
