摘要
目的探讨妊娠期高血压疾病(HDCP)对新生儿脑功能及神经行为发育的影响。方法选择2017年8月至2019年8月在邢台市人民医院出生的母孕期合并HDCP的新生儿80例作为研究组,另选择同期健康母亲所生新生儿80例作为对照组,对其临床资料进行回顾性分析。比较2组新生儿在出生后不同时间的局部脑组织氧饱和度(rScO_(2))、新生儿20项行为神经评分(NBNA)、全身运动(GMs)质量评估结果及Peabody运动发育量表(PDMS)评分。结果研究组新生儿出生24、48、72 h的rScO_(2)均显著低于对照组(P<0.05)。研究组新生儿出生42 d的NBNA评分显著低于对照组,新生儿神经行为异常患儿占比显著高于对照组(P<0.05)。研究组新生儿出生30 d、2个月和3个月的GMs异常占比均显著高于对照组(P<0.05)。研究组新生儿出生6、12、18个月的PDMS评分均显著低于对照组(P<0.05)。结论合并HDCP的孕妇所生新生儿脑组织氧合代谢较差,新生儿中枢神经损害及神经行为异常比例均较高。
Objective To investigate the effect of hypertension disorders complicating pregnancy(HDCP)on brain function and neurobehavioral development of newborns.Methods A total of 80 newborns born to pregnant women with HDCP at Xingtai People′s Hospital from August 2017 to August 2019 were selected as the study group,and another 80 newborns born to healthy mothers during the same period were selected as the control group,their clinical data were retrospective analyzed.The cerebral regional oxygen saturation(rScO_(2)),neonatal behavioral neurological assessment(NBNA),general movements(GMs)quality assessment and Peabody developmental motor scale(PDMS)of newborns at different times after birth were compared between two groups.Results The rScO_(2)levels of newborns at 24,48,72 h of birth in the study group were significantly lower than those in the control group(P<0.05).The NBNA score of newborns at 42 days of birth in the study group was significantly lower than that in the control group,and the proportion of newborns with neurobehavioral abnormalities was significantly higher than that in the control group(P<0.05).The proportion of abnormal GMs of newborns at 30 days,2 months,and 3 months after birth in the study group was significantly higher than that in the control group(P<0.05).The PDMS scores of newborns at 6,12,18 months after birth in the study group were significantly lower than those in the control group(P<0.05).Conclusion Newborns born to pregnant women with HDCP have poor brain tissue oxygenation metabolism,and a higher proportion of central nervous system damage and neurobehavioral abnormalities.
