摘要
目的:探讨健康体适能评估联合综合干预对代谢综合征(MetS)相关代谢指标的影响,为优化MetS健康管理模式提供科学依据。方法:采用前瞻性队列研究设计,纳入2022年1月至12月于贵州医科大学附属医院体检中心的392例受试者,分为干预组(接受健康体适能评估及个性化运动与健康教育)和对照组(常规体检)。通过倾向性评分匹配衡基线差异,对比两组6~12个月后体重、血脂、血糖等代谢指标的变化,采用线性回归分析干预措施的影响效应。结果:干预组体质量指数(BMI)由24.2 kg/m^(2)降至22.8 kg/m^(2)(差值1.44 kg/m^(2),P=0.001),超重肥胖率从54.2%降至36.1%(P=0.001);对照组BMI及超重肥胖率无显著变化。干预组低密度脂蛋白胆固醇(LDL-C)降低0.13 mmol/L(P=0.002),而对照组升高0.09 mmol/L(P<0.001);空腹血糖(FBG)干预组下降0.26 mmol/L(P<0.001),对照组下降0.14 mmol/L(P<0.001)。多因素模型显示,干预措施显著改善BMI(调整后差值1.56 kg/m^(2))、LDL-C(0.18 mmol/L)和FBG(0.18 mmol/L)(均P<0.05)。结论:基于健康体适能评估的个体化干预可有效改善体重、血脂及血糖代谢,为Met S的体医融合管理模式提供了数据支持。
Objective:This study aims to investigate the impact of health-related physical fitness assessments combined with comprehensive interventions on metabolic indicators associated with metabolic syndrome(MetS),thereby providing scientific evidence for optimizing the health management model of Mets.Method:A prospective cohort study design was employed,enrolling 392 subjects from the Health Examination Center of Guizhou Medical University Affiliated Hospital between January and December 2022.Participants were divided into an intervention group,which received health-related physical fitness assessments along with personalized exercise and health education,and a control group,which underwent routine health examinations.Baseline differences between the groups were balanced using propensity score matching.Changes in metabolic indicators,including body weight,blood lipids,and blood glucose,were compared between the two groups after 6 to 12 months.The effects of the intervention measures were analyzed using linear regression.Result:In the intervention group,the Body Mass Index(BMI)decreased from 24.2 kg/m^(2)to 22.8 kg/m^(2)(difference of 1.44 kg/m^(2),P-0.001),and the rate of overweight and obesity decreased from 54.2%to 36.1%(P-0.001).No significant changes in BMI or the rate of overweight and obesity were observed in the control group.Additionally,LDL-C in the intervention group decreased by 0.13 mmol/L(P-0.002),while it increased by 0.09 mmol/L in the control group(P<0.001).Fasting blood glucose(FBG)decreased by 0.26 mmol/L in the intervention group(P<0.001)and by 0.14 mmol/L in the control group(P<0.001).The multifactorial model indicated that the intervention significantly improved BMI(adjusted difference of 1.56 kg/m^(2)),LDL-C(0.18 mmol/L),and FBG(0.18 mmol/L)(all P<0.05).Conclusion:Individualized interventions based on health-related physical ftness assessments can effectively improve body weight,lipid profiles,and glucose metabolism,thereby providing data support for the integrated management model of exercise and medicine for MetS.
