血清MIF、Neopterin、LRG1与糖尿病合并肺结核患者预后不良的关系

Study on the relationship between serum MIF,Neopterin,LRG1 and poor prognosis in patients with diabetes mellitus combine with pulmonary tuberculosis

摘要 目的检测糖尿病合并肺结核患者血清巨噬细胞移动抑制因子(MIF)、新喋呤(Neopterin)、富亮氨酸α2-糖蛋白1(LRG1)水平,分析其与糖尿病合并肺结核患者预后不良的关系。方法选取2021年10月-2022年10月期间重庆市开州区人民医院收治的糖尿病合并肺结核患者145例,患者均采用标准抗结核方案治疗,治疗6个月后评估患者的预后情况,根据不同预后情况分为预后不良组和预后良好组。对比预后不良组与预后良好组一般资料及血清MIF、Neopterin、LRG1水平。采用受试者工作特征(ROC)曲线分析血清MIF、Neopterin、LRG1对糖尿病合并肺结核患者预后不良的预测价值。结果与预后良好组比较,预后不良组血清MIF、Neopterin、LRG1水平升高,差异均有统计学意义(t=8.810、7.549、4.521,P均<0.05)。预后不良组非初治、耐药、合并空洞、糖化血红蛋白>7%占比均高于预后良好组(χ^(2)=5.128、5.595、4.193、7.158,P均<0.05),年龄≥60岁、使用免疫增强剂占比低于预后良好组(χ^(2)=5.786、8.588,P均<0.05),差异均有统计学意义。ROC分析结果显示,血清MIF、Neopterin、LRG1单独检测预测糖尿病合并肺结核患者预后不良的曲线下面积(AUC)分别为0.776、0.807、0.755,三者联合预测的AUC为0.922。结论血清MIF、Neopterin、LRG1水平在糖尿病合并肺结核患者中升高,与患者预后不良密切相关,三者联合检测对糖尿病合并肺结核预后不良有较高的预测价值。 Objective To investigate the relationship between serum macrophage migration inhibitory factor(MIF),neopterin(Neopterin),leucine-rich α2-glycoprotein 1(LRG1)and poor prognosis in patients with diabetes mellitus combine with pulmonary tuberculosis.Methods A total of 145 patients with diabetes mellitus combine with pulmonary tuberculosis admitted to the Chongqing Kaizhou District People's Hospital from October 2021 to October 2022 were selected;all patients were treated with standard anti-tuberculosis regimen;the prognosis of patients was evaluated 6 months after treatment;patients were divided into poor prognosis group and good prognosis group according to different prognosis.The general information and the serum levels of MIF,neopterin and LRG1 were compared between poor prognosis group and good prognosis group.The predictive value of serum MIF,neopterin and LRC1 for poor prognosis in patients with diabetes mellitus combine with pulmonary tuberculosis was analyzed by receiver operating characteristic(ROC)curve.Results Compared with good prognosis group,the serum levels of MIF,neopterin and LRG1 in poor prognosis group were increased(t=8.810,7.549,4.521;all P<0.05).The proportion of non-initial treatment,drug resistance,combined cavity and glycosylated hemoglobin>7% in poor prognosis group was higher than those in good prognosis group(χ^(2)=5.128,5.595,4.193,7.158;all P<0.05),and the proportion of age≥60 years old and the use of immunopotentiators was lower than those in good prognosis group(χ^(2)=5.786,8.588;all P<0.05).The results of ROC analysis showed that,the areas under the curve(AUC)of serum MIF,neopterin and LRGI alone in predicting the poor prognosis of patients with diabetes mellitus combine with pulmonary tuberculosis were 0.776,0.807 and 0.755,respectively;the AUC of the combined prediction of the three indicators was 0.922.Conclusion The levels of serum MIF,neopterin and LRG1 in patients with diabetes mellitus combine with pulmonary tuberculosis were elevated,which were closely related to the poor prognosis of patients;the combine detection of the three had a high predictive value for the poor prognosis of diabetes mellitus combine with pulmonary tuberculosis.
