摘要
目的探讨痰培养联合尿肺炎链球菌抗原检测诊断儿童细菌性肺炎的临床价值。方法回顾性抽取2020年1月至2023年12月中国人民解放军联勤保障部队第九八七医院收治的细菌性肺炎患儿115例。采集患儿痰样本、尿液样本, 对痰样本病原体进行分离培养, 采用免疫层析法测定尿样本中肺炎链球菌抗原, 分析痰培养联合尿肺炎链球菌抗原对细菌性肺炎的诊断价值。结果 115例痰标本和尿标本中, 痰培养阳性率(21.7%, 25/115)低于尿肺炎链球菌抗原阳性率(33.9%, 39/115), P<0.05。痰培养阳性患者尿肺炎链球菌抗原阳性率(92.0%, 23/25)高于痰培养阴性患者(17.8%, 16/90), P<0.05。尿肺炎链球抗原检测诊断儿童细菌性肺炎的灵敏度、特异度、准确度分别为92.0%、82.2%、84.3%, 两者联合的阳性检出率为35.7%。结论尿肺炎链球菌抗原检测在肺炎链球菌感染引发的儿童细菌性肺炎中具有较高的诊断价值, 与痰培养联合可提高阳性检出率。
ObjectiveTo investigate the clinical value of sputum culture combined with Streptococcus pneumoniae urinary antigen testing in the diagnosis of bacterial pneumonia in children.MethodsA retrospective study was conducted on 115 children with bacterial pneumonia admitted to No.987 Hospital of the Joint Logistic Support Force of the Chinese People’s Liberation Army from January 2020 to December 2023.The sputum samples and urine samples were collected from the 115 children,and the pathogens in the sputum samples were isolated and cultured.The Streptococcus pneumoniae antigen in the urine samples was tested by immunochromatography,and the diagnostic value of sputum culture combined with Streptococcus pneumoniae urinary antigen testing for bacterial pneumonia was analyzed.ResultsAmong the 115 sputum samples and 115 urine samples,the positive rate of Streptococcus pneumoniae in sputum culture(21.7%,25/115)was lower than that of Streptococcus pneumoniae urinary antigen testing(33.9%,39/115),P<0.05.The positive rate of Streptococcus pneumoniae urinary antigen in patients with positive sputum culture(92.0%,23/25)was higher than that in patients with negative sputum culture(17.8%,16/90),P<0.05.The sensitivity,specificity and accuracy of Streptococcus pneumoniae urinary antigen testing in the diagnosis of bacterial pneumonia in children were 92.0%,82.2%and 84.3%,respectively.The positive rate of combination of the two methods was 35.7%.ConclusionsUrinary antigen test has a high diagnostic value for bacterial pneumonia caused by Streptococcus pneumoniae infection in children;moreover,Streptococcus pneumoniae urinary antigen test combined with sputum culture can improve the positive detection rate.
