单水平气道正压通气对OSAHS相关性高血压患者miRNA-126作用的研究

摘要 目的探讨单水平气道正压通气(CPAP)对阻塞性睡眠呼吸暂停低通气综合征(OSAHS)相关性高血压患者miRNA-126的作用。方法选择OSAHS相关性高血压患者25例,收集患者一般资料、睡眠监测(PSG)数据、24h动态血压监测结果,比较CPAP治疗4周前后采集外周血检测miRNA-126、血管内皮生长因子(VEGF)、缺氧诱导因子-1α(HIF-1α)、肿瘤坏死因子-α(TNF-α)的变化。结果治疗后呼吸暂停低通气指数(AHI)、氧减指数(ODI)、血氧饱和度<90%时间较治疗前下降;平均血氧饱和度(MSpO_(2))、最低血氧饱和度(LSpO_(2))较治疗前增高(P<0.05)。患者白天收缩压与舒张压均值、夜间收缩压与舒张压均值、全天收缩压与舒张压均值治疗后均较治疗前降低(P<0.05)。治疗后miRNA-126水平较治疗前升高,HIF-1α、VEGF、TNF-α较治疗前下降(P<0.05)。治疗前miRNA-126与HIF-1α、VEGF、TNF-α均呈负相关(P<0.05)。结论CPAP治疗后,OSAHS相关性高血压患者呼吸事件减少,缺氧改善,血压指标较治疗前下降。CPAP治疗后,miRNA-126较治疗前升高,相关炎症因子水平下降,miRNA-126可能影响OSAHS相关性高血压的发生。 Objective To explore the effect of single-level positive airway pressure ventilation(CPAP)on miRNA-126 in patients with obstructie sleep apnea-hypopnea syndrome(OSAHS)-related hyperttension.Methods 25 patients with OSAHS-related hypertension were recruited,and the general information,polysomnography(PSG)data,and 24-hour ambulatory blood pressure monitoring results were collected.The changes of miRNA-126,vascular endothelial growth factor(VEGF),hypoxia-inducible factor-1α(HIF-1α),tumor necrosis factor-α(TNF-α)in peripheral blood were detected before and after 4 weeks of treatment with continuous positive airway pressure(CPAP).Results Compared to before treatment,the apnea-hypopnea index(AHI),oxygen desaturation index(ODI),and time with blood oxygen saturation<90%were decreased after treatment,the mean blood oxygen saturation(MSpO_(2))and the lowest blood oxygen saturation(LSpO_(2))were increased after treatment(P<0.05).The daytime systolic blood pressure,daytime diastolic blood pressure,nighttime systolic blood pressure,nighttime diastolic blood pressure,average systolic blood pressure,average diastolic blood pressure were all decreased after CPAP compared to before treatment(P<0.05).The level of miRNA-126 increased,while the levels of HIF-1α,VEGF,and TNF-αdecreased after treatment compared to before treatment(P<0.05).Before treatment,miRNA-126 was negatively correlated with HIF-1α,VEGF,and TNF-α(P<0.05).Conclusions After CPAP therapy,the patients with OSAHS-related hypertension had fewer respiratory events,improved hypoxia,and lower systolic and diastolic blood pressure.After CPAP therapy,the level of miRNA-126 increased and the the levels of related inflammatory factors decreased.MiRNA-126 may affect the occurrence of OSAHS-related hypertension.
